Abstract

Surabaya, Gresik, dan Sidoarjo merupakan kawasan metropolitan terintegrasi dengan total populasi lebih dari 6 juta jiwa dan mobilitas penduduk yang tinggi melalui aktivitas industri dan perdagangan. Tingginya kepadatan penduduk dan interaksi sosial yang intens di wilayah ini meningkatkan kerentanan terhadap tantangan kesehatan masyarakat, khususnya penyebaran kasus HIV/AIDS. Peningkatan kasus ini memerlukan pemodelan statistik yang akurat untuk memahami pola hubungan nonlinear dan interaksi kompleks antar faktor yang memengaruhinya. Penelitian ini bertujuan untuk memodelkan jumlah kasus HIV/AIDS di Kota Surabaya, Kabupaten Gresik, dan Kabupaten Sidoarjo menggunakan pendekatan Bootstrap Aggregating Multivariate Adaptive Regression Splines (Bagging MARS). Metode MARS digunakan karena fleksibilitasnya dalam menangani data berdimensi tinggi tanpa asumsi distribusi, sementara teknik Bagging diterapkan untuk meningkatkan stabilitas dan akurasi prediksi model. Data yang dianalisis merupakan data sekunder tahun 2024 dari Publikasi Dinas Kesehatan Surabaya, Dinas Kesehatan Gresik, dan Dinas Kesehatan Sidoarjo dengan variabel respon persentase kasus HIV/AIDS per kecamatan. Variabel prediktor mencakup faktor predisposisi dan faktor pemungkin berdasarkan kerangka teori Lawrence Green, seperti tenaga kesehatan, penduduk usia subur, jenis kelamin, tingkat pendidikan, dan penggunaan alat kontrasepsi. Model MARS yang terbaik ditentukan berdasarkan kriteria GCV minimum dan diperoleh model terbaik dari hasil analisis adalah model dengan fungsi basis (BF), Maximum Interaction (MI), dan Minimum Observation (MO) berturut-turut sebesar 20, 3, dan 3 dengan nilai GCV sebesar 0,000003567. Variabel prediktor yang paling berpengaruh terhadap model adalah variabel persentase penduduk yang menggunakan alat kontrasepsi kondom dan variabel persentase tenaga kesehatan. Perbandingan antara metode MARS dan Bagging MARS didapatkan kesimpulan bahwa metode terbaik untuk persentase kasus baru HIV/AIDS di Surabaya, Gresik, dan Sidoarjo tahun 2024 adalah metode Bagging MARS dengan jumlah replikasi sebanyak 50 kali karena menghasilkan nilai GCV terkecil yaitu sebesar 0,0000008479459.

Surabaya, Gresik, and Sidoarjo are integrated metropolitan areas with a total population of more than six million people and high population mobility driven by industrial and commercial activities. The high population density and intensive social interactions in these regions increase vulnerability to public health challenges, particularly the spread of HIV/AIDS. The increasing number of HIV/AIDS cases requires accurate statistical modeling to understand the nonlinear relationships and complex interactions among influencing factors. This study aims to model the percentage of new HIV/AIDS cases in Surabaya City, Gresik Regency, and Sidoarjo Regency using the Bootstrap Aggregating Multivariate Adaptive Regression Splines (Bagging MARS) approach. The MARS method was employed due to its flexibility in handling high-dimensional data without distributional assumptions, while the Bagging technique was applied to improve model stability and predictive accuracy. The data analyzed were secondary data from the 2024 publications of the Health Offices of Surabaya, Gresik, and Sidoarjo, with the response variable being the percentage of HIV/AIDS cases at the subdistrict level. The predictor variables consisted of predisposing and enabling factors based on Lawrence Green’s theoretical framework, including healthcare personnel, productive-age population, gender, educational attainment, and contraceptive use. The best MARS model was selected based on the minimum Generalized Cross Validation (GCV) criterion and was obtained with 20 Basis Functions (BF), a Maximum Interaction (MI) of 3, and a Minimum Observation (MO) of 3, resulting in a GCV value of 0.000003567. The most influential predictor variables in the model were the percentage of condom users and the percentage of healthcare personnel. The comparison between the MARS and Bagging MARS methods indicated that Bagging MARS with 50 bootstrap replications provided the best performance, yielding the smallest GCV value of 0.0000008479459. Therefore, Bagging MARS was found to be more effective in modeling the percentage of new HIV/AIDS cases in Surabaya, Gresik, and Sidoarjo in 2024.
