Abstract

Penelitian ini mengembangkan mikropartikel biodegradable berbasis PLGA yang mengandung lidocaine dan ketorolac menggunakan teknik electrospray sebagai sistem penghantaran obat lepas lambat untuk terapi osteoartritis sendi faset lumbosakral. Formulasi optimum terdiri atas 300 mg PLGA dan 75 mg obat yang dilarutkan dalam 5 mL DCM, kemudian diproses menggunakan electrospray pada tegangan 15.000 V dengan laju alir 0,8 mL/jam. Hasil FE-SEM menunjukkan bahwa mikropartikel memiliki morfologi berbentuk sferis dengan permukaan yang halus dan terdispersi secara merata. Nilai potensial zeta sebesar −37,63 mV pada PLGA bermuatan lidocaine dan −31,45 mV pada PLGA bermuatan ketorolac menunjukkan stabilitas partikel yang baik. Analisis FTIR dan DSC mengonfirmasi bahwa kedua obat berhasil dienkapsulasi dan terdispersi ke dalam matriks PLGA tanpa mengubah struktur kimia khas PLGA. Uji pelepasan obat secara in vitro selama 30 hari menunjukkan pelepasan obat yang berkelanjutan, dengan pelepasan kumulatif sebesar 92,16% untuk lidocaine dan 37,23% untuk ketorolac. Uji sitotoksisitas menunjukkan biokompatibilitas yang baik. Pengujian in vivo lebih lanjut mengonfirmasi pelepasan obat secara lokal yang berkelanjutan, dengan kedua obat masih terdeteksi pada jaringan di sekitar lokasi injeksi hingga 14 hari, sedangkan hanya ditemukan konsentrasi obat yang sangat rendah di dalam darah. Analisis histologi menunjukkan proses penyembuhan jaringan yang normal tanpa adanya respons inflamasi berkepanjangan. Secara keseluruhan, mikropartikel PLGA yang dikembangkan memiliki karakteristik fisikokimia yang baik, kemampuan pelepasan obat secara berkelanjutan, biokompatibilitas yang tinggi, serta kompatibilitas jaringan yang baik, sehingga berpotensi sebagai sistem penghantaran obat lokal untuk manajemen nyeri pascaoperasi dan terapi osteoartritis sendi faset lumbosakral.

This study developed biodegradable microparticles based PLGA containing lidocaine and ketorolac using the electrospray technique as a sustained drug delivery system for the treatment of lumbosacral facet joint osteoarthritis. The optimized formulation consisted of 300 mg PLGA and 75 mg drug dissolved in 5 mL DCM, electrosprayed at 15,000 V with a flow rate of 0.8 mL/h. Microparticles were characterized using FE-SEM, particle size analysis, zeta potential, FTIR, and DSC. FE-SEM images. Zeta potential values of −37.63 mV for lidocaine-loaded PLGA and −31.45 mV for ketorolac-loaded PLGA indicated good particle stability. FTIR analysis confirmed the successful encapsulation and successfully dispersed of both drugs while maintaining the characteristic chemical structure of PLGA. The in vitro release study demonstrated sustained drug release over 30 days, with cumulative releases of 92.16% for lidocaine and 37.23% for ketorolac. Cytotoxicity testing showed that the released drugs indicating good biocompatibility. In vivo studies further confirmed sustained local drug release, with both drugs remaining detectable in tissues around the injection site for up to 14 days while only negligible drug concentrations were found in the blood. Histological analysis showed normal tissue healing without prolonged inflammation. Overall, the developed PLGA microparticles demonstrated favorable physicochemical properties, sustained drug release, good biocompatibility, and good tissue compatibility, indicating their potential as a localized sustained drug delivery system for postoperative pain management and the treatment of lumbosacral facet joint osteoarthritis.
