Abstract
Occupational Safety and Health (OSH) risk management is a critical concern in manufacturing industries because production activities involving machines, welding, cutting, grinding, and material handling carry a high potential for workplace accidents. PT Widjaya Teknik Indonesia, a metal-based manufacturing company producing stainless steel products, faces various operational risks that can affect worker safety and production continuity, yet has never conducted a formal OSH risk assessment. This study applies the HORshe (House of Risk for Safety and Health Environment) method, which integrates the House of Risk model, the SHELL model, and the Hierarchy of Controls, to identify risk events, determine priority risk agents, and formulate preventive actions. The identification process found 20 Risk Events, dominated by the Liveware-Hardware category (12 events, 60%), and 20 underlying Risk Agents. Aggregate Risk Potential (ARP) calculation showed that Risk Agent A1 (absence of a dedicated Occupational Safety and Health function) contributed the largest share of total risk at 25.4%, followed by A3 (absence of routine training and inaccessible SOP documentation) at 17.6%, together accounting for 43.1% of total ARP. Applying the 80% Pareto threshold identified nine priority Risk Agents, all related to organizational, procedural, or PPE selection gaps rather than equipment condition. Sixteen Preventive Actions were formulated and classified using the Hierarchy of Controls, comprising one Elimination, two Substitution, four Engineering Control, eight Administrative Control, and one Personal Protective Equipment action, with PA1 (outsourcing in house blade sharpening) ranked as the top priority under both the Hierarchy of Controls ordering and the Effectiveness to Difficulty Ratio (ETD). A financial feasibility analysis using a 60% Expected Reduction Percentage reference scenario produced a Benefit Cost Ratio of 1.23x in Year 1 and 3.93x in the Steady State period, both indicating the proposed mitigation strategy is financially feasible. These findings are expected to serve as a practical basis for PT Widjaya Teknik Indonesia in prioritizing OSH improvement investment

Manajemen risiko Keselamatan dan Kesehatan Kerja (K3) merupakan aspek penting dalam industri manufaktur karena kegiatan produksi yang melibatkan mesin, pengelasan, pemotongan, penggerindaan, dan penanganan material memiliki potensi tinggi terjadinya kecelakaan kerja. PT Widjaya Teknik Indonesia, perusahaan manufaktur berbasis logam yang memproduksi produk baja tahan karat, menghadapi berbagai risiko operasional yang dapat memengaruhi keselamatan pekerja dan kelangsungan produksi, namun belum pernah melakukan penilaian risiko K3 secara formal. Penelitian ini menerapkan metode HORshe (House of Risk for Safety and Health Environment) yang mengintegrasikan model House of Risk, model SHELL, dan Hierarchy of Controls untuk mengidentifikasi kejadian risiko, menentukan agen risiko prioritas, dan merumuskan tindakan pencegahan. Proses identifikasi menemukan 20 Kejadian Risiko yang didominasi kategori Liveware-Hardware (12 kejadian, 60%), serta 20 Agen Risiko penyebabnya. Perhitungan Aggregate Risk Potential (ARP) menunjukkan bahwa Agen Risiko A1 (tidak adanya fungsi K3 khusus) menyumbang porsi risiko terbesar sebesar 25,4%, diikuti A3 (tidak adanya pelatihan rutin dan dokumentasi SOP yang sulit diakses) sebesar 17,6%, sehingga keduanya menyumbang 43,1% dari total ARP. Penerapan ambang Pareto 80% menghasilkan sembilan Agen Risiko prioritas yang seluruhnya berkaitan dengan celah organisasi, prosedural, atau pemilihan APD, bukan kondisi fisik mesin. Sebanyak lima belas Tindakan Pencegahan dirumuskan dan diklasifikasikan menggunakan Hierarchy of Controls, terdiri dari satu tindakan Eliminasi, dua Substitusi, tiga Engineering Control, delapan Administrative Control, dan satu Alat Pelindung Diri, dengan PA1 (outsourcing pengasahan pisau) menempati prioritas tertinggi baik berdasarkan urutan Hierarchy of Controls maupun rasio Effectiveness to Difficulty (ETD). Analisis kelayakan finansial dengan skenario referensi Expected Reduction Percentage sebesar 60% menghasilkan Benefit Cost Ratio sebesar 1,23x pada Tahun 1 dan 3,93x pada periode Steady State, yang menunjukkan strategi mitigasi yang diusulkan layak secara finansial. Temuan ini diharapkan dapat menjadi dasar praktis bagi PT Widjaya Teknik Indonesia dalam memprioritaskan investasi perbaikan K3.
