Abstract

The continuous growth of port and industrial maritime operations requires effective safety management to minimize the risk of accidents during berthing operations. At PT X (name anonymized) Private Jetty, Surabaya, several berthing incidents involving bulk carriers have resulted in vessel-jetty contact, fender damage, operational disruption, and significant repair costs, highlighting the need for a systematic risk assessment. This study aims to analyze berthing risks using the Formal Safety Assessment (FSA) framework, supported by Failure Mode and Effect Analysis (FMEA) for hazard identification and Cost Benefit Assessment (CBA) for evaluating the economic feasibility of the proposed Risk Control Options (RCOs). Data were collected through field observations, historical accident records, incident reports, and interviews with pilots and jetty operators. The assessment identified nineteen hazards associated with berthing operations. Based on hazard prioritization, excessive approach speed, delayed maneuver execution, adverse weather conditions, and human error were identified as the Priority Hazards requiring immediate mitigation. Four Risk Control Options (RCOs) were subsequently developed and evaluated using semi-quantitative Cost Benefit Assessment. The results indicate that all proposed RCOs provide positive economic benefits, as the estimated avoided losses exceed the implementation costs. The findings demonstrate that integrating FSA, FMEA, and CBA provides a systematic framework for risk-informed decision-making and improving berthing safety at private jetties.

Pertumbuhan kegiatan pelabuhan dan operasi maritim yang terus meningkat menuntut penerapan manajemen keselamatan yang efektif untuk meminimalkan risiko kecelakaan selama proses sandar kapal. Di Pelabuhan Privat PT X (nama disamarkan), Surabaya, beberapa kejadian sandar yang melibatkan kapal curah mengakibatkan benturan antara kapal dan dermaga, kerusakan fender, gangguan operasional, serta kerugian finansial yang signifikan sehingga diperlukan suatu pendekatan penilaian risiko yang sistematis. Penelitian ini bertujuan menganalisis risiko operasi sandar dengan menerapkan kerangka Formal Safety Assessment (FSA), menggunakan Failure Mode and Effect Analysis (FMEA) sebagai alat pendukung identifikasi bahaya, serta Cost Benefit Assessment (CBA) untuk mengevaluasi kelayakan ekonomis dari Risk Control Options (RCOs) yang diusulkan. Data penelitian diperoleh melalui observasi lapangan, data historis kecelakaan, laporan insiden, dan wawancara dengan pandu serta operator dermaga. Hasil penelitian mengidentifikasi sembilan belas (19) bahaya pada operasi sandar. Berdasarkan proses prioritisasi bahaya, excessive approach speed, delayed maneuver execution, adverse weather conditions, dan human error ditetapkan sebagai Priority Hazards yang memerlukan perhatian utama dalam upaya mitigasi risiko. Selanjutnya, empat Risk Control Options (RCOs) dikembangkan dan dievaluasi menggunakan Cost Benefit Assessment (CBA) semi-kuantitatif. Hasil evaluasi menunjukkan bahwa seluruh RCO yang diusulkan memberikan manfaat ekonomi positif karena estimasi kerugian yang dapat dihindari lebih besar dibandingkan biaya implementasinya. Dengan demikian, integrasi FSA, FMEA, dan CBA memberikan pendekatan yang sistematis dalam mendukung pengambilan keputusan berbasis risiko serta meningkatkan keselamatan operasi sandar pada pelabuhan privat.
