Abstract

Tailing merupakan sisa hasil pengolahan bijih logam yang membutuhkan pengelolaan cermat dari aspek teknis, lingkungan, dan regulasi. Di Tambang Emas Martabe, PT Agincourt Resources (PTAR) saat ini mengoperasikan fasilitas Tailings Storage Facility (TSF) dengan kapasitas penampungan yang diproyeksikan hanya mencukupi hingga tahun 2028. Untuk mengantisipasi kebutuhan jangka panjang, perusahaan merencanakan pembangunan fasilitas baru dengan metode penimbunan filtered tailings (dry stack) berkapasitas 41 juta ton. Metode ini menawarkan keunggulan teknis dan lingkungan, tetapi juga menimbulkan potensi risiko kegagalan yang perlu diidentifikasi dan dikelola secara sistematis. Penelitian ini bertujuan untuk menilai risiko pada fasilitas filtered tailings dengan pendekatan Failure Mode and Effect Analysis (FMEA) yang dikombinasikan dengan Fault Tree Analysis (FTA). Analisis dilakukan melalui identifikasi mode kegagalan potensial, penilaian tingkat keparahan, probabilitas kejadian, kemampuan deteksi, serta perhitungan Risk Priority Number (RPN) untuk menentukan prioritas dan menyusun respons risiko. Data diperoleh melalui studi literatur, penyebaran kuesioner, serta Focus Group Discussion (FGD) bersama tim ahli di bidang tailing, sehingga hasil analisis mencerminkan kondisi di lapangan. Hasil penelitian menunjukkan bahwa terdapat 24 mode kegagalan yang teridentifikasi pada sistem filtered tailings. Berdasarkan analisis FMEA, 6 mode kegagalan memiliki nilai RPN tinggi sehingga menjadi prioritas pengendalian. Dari analisis FTA, didapatkan tiga klaster penyebab kegagalan fasilitas filtered tailings, yaitu geoteknik, hidrologi dan drainase, serta lingkungan dan geokimia. Setelah penerapan strategi respons risiko, nilai RPN residual mengalami penurunan signifikan hingga berada pada kategori risiko yang dapat diterima. Hasil penelitian ini menghasilkan rekomendasi respons risiko yang dapat digunakan sebagai masukan dalam dokumen desain, spesifikasi teknis, risk register, serta sistem manajemen risiko perusahaan, sehingga mendukung pengembangan praktik pertambangan yang aman, efisien, dan berkelanjutan.

Tailings are residual materials generated from mineral ore processing that require careful management from technical, environmental, and regulatory perspectives. At the Martabe Gold Mine, PT Agincourt Resources (PTAR) currently operates a Tailings Storage Facility (TSF), with its storage capacity projected to be sufficient only until 2028. To anticipate long-term tailings management needs, the company plans to develop a new facility using the filtered tailings (dry stack) method with a capacity of 41 million tonnes. This method offers technical and environmental advantages; however, it also presents potential failure risks that need to be systematically identified and managed. This study aims to assess risks associated with the filtered tailings facility using the Failure Mode and Effect Analysis (FMEA) approach combined with Fault Tree Analysis (FTA). The analysis was conducted through the identification of potential failure modes, assessment of severity, occurrence, and detection, followed by the calculation of the Risk Priority Number (RPN) to determine risk priorities and develop risk responses. Data were collected through literature studies, questionnaires, and Focus Group Discussions (FGD) involving experts in tailings management, ensuring that the analysis represents actual field conditions. The results indicate that 24 failure modes were identified in the filtered tailings system. Based on the FMEA analysis, six failure modes obtained high RPN values and were prioritized for risk control. The FTA analysis identified three major failure cause clusters in the filtered tailings facility, namely geotechnical, hydrology and drainage, and environmental and geochemical aspects. Following the implementation of risk response strategies, the residual RPN values decreased significantly and reached an acceptable risk category. The results of this study provide risk response recommendations that can be used as inputs for design documents, technical specifications, risk registers, and corporate risk management systems, thereby supporting the development of safer, more efficient, and sustainable mining practices.
