Abstract

Penerapan standar Health Level Seven (HL7) dan Fast Healthcare Interoperability Resources (FHIR) untuk meningkatkan interoperabilitas data antara Sistem Informasi Manajemen Rumah Sakit (SIMRS) dan sistem BPJS Kesehatan dalam konteks implementasi Rekam Medis Elektronik (RME). Interoperabilitas data kesehatan menjadi tantangan kritis dalam ekosistem layanan kesehatan Indonesia, terutama dalam pertukaran data antara provider layanan kesehatan dan penjamin. Ketidakseragaman format data, inkonsistensi terminologi medis, dan fragmentasi sistem informasi kesehatan menyebabkan inefisiensi operasional, duplikasi data, dan hambatan dalam kontinuitas layanan kesehatan. Penelitian ini menggunakan pendekatan kualitatif dengan metode studi literatur, analisis dokumen standar, dan evaluasi implementasi sistem yang ada. Standar HL7 dan FHIR dipilih karena sifatnya yang modern, berbasis RESTful API, dan menggunakan format data yang mudah diimplementasikan (JSON/XML). Analisis mencakup evaluasi arsitektur sistem, pemetaan proses pertukaran data, identifikasi resource FHIR yang relevan (Patient, Practitioner, Encounter, Observation, Medication, dll), serta analisis kesiapan digital infrastruktur kesehatan Indonesia. Temuan empiris menunjukkan bahwa tingkat eror data dasar saat ini mencapai 18% hingga 22% selama proses pertukaran antar-sistem, yang utamanya dipicu oleh kolom data yang belum terstandardisasi. Penilaian terhadap infrastruktur digital kesehatan nasional menghasilkan skor kesiapan FHIR sebesar 2,9 dari 5, yang mengindikasikan kesiapan teknis tingkat moderat dengan celah kritis pada integrasi semantik. Lebih lanjut, evaluasi keamanan data menunjukkan tingkat kepatuhan sebesar 58,8% terhadap standar ISO 27001. Penelitian ini memberikan contoh penerapan HL7 dan FHIR dalam melakukan pertukaran data dalam upaya meningkatkan akurasi verifikasi kepesertaan secara real-time. Penelitian ini juga mengidentifikasi tantangan implementasi yang meliputi kesiapan infrastruktur teknologi, standardisasi terminologi medis Systematized Nomenclature of Medicine – Clinical Terms (SNOMED-CT) dan International Statistical Classification of Diseases and Related Health Problems, 10th Revision (ICD-10), keamanan dan privasi data, serta aspek regulasi dan tata kelola (governance). Penelitian ini berkontribusi pada pengembangan interoperabilitas SIMRS dan BPJS Kesehatan dalam ruang lingkup nasional yang dapat menjadi acuan bagi pemangku kepentingan (stakeholder) kesehatan Indonesia dalam mewujudkan ekosistem kesehatan digital yang terintegrasi, efisien, dan berpusat pada pasien.

The implementation of Health Level Seven (HL7) and Fast Healthcare Interoperability Resources (FHIR) standards to improve data interoperability between Hospital Management Information Systems (SIMRS) and the BPJS Kesehatan system within the context of Electronic Medical Record (RME) implementation. Healthcare data interoperability is a critical challenge in Indonesia's healthcare ecosystem, particularly regarding data exchange between healthcare providers and payers. Discrepancies in data formats, inconsistencies in medical terminology, and the fragmentation of health information systems lead to operational inefficiencies, data duplication, and barriers to continuity of care. This study employs a qualitative approach utilizing literature reviews, document analysis of standards, and implementation evaluations of existing systems. The HL7 and FHIR standards were selected due to their modern, RESTful API-based nature and their utilization of easily implementable data formats (JSON/XML). The analysis encompasses evaluations of system architecture, mapping of data exchange processes, identification of relevant FHIR resources (Patient, Practitioner, Encounter, Observation, Medication, etc.), and an assessment of the digital readiness of Indonesia's healthcare infrastructure. The empirical findings reveal a current baseline data error rate of 18% to 22% during inter-system exchange, driven primarily by unstandardized fields. Assessment of the national healthcare digital infrastructure yields a FHIR readiness score of 2.9 out of 5, indicating moderate technical preparedness with critical gaps in semantic integration. Furthermore, data security evaluations show a 58.8% compliance rate with the ISO 27001 standard. This study provides practical examples of HL7 and FHIR implementation in data exchange to enhance the accuracy of real-time membership verification. Furthermore, this research identifies key implementation challenges, including technological infrastructure readiness, the standardization of medical terminologies such as the Systematized Nomenclature of Medicine – Clinical Terms (SNOMED-CT) and the International Statistical Classification of Diseases and Related Health Problems, 10th Revision (ICD-10), data security and privacy, as well as regulatory and governance aspects. This study contributes to the development of interoperability between SIMRS and BPJS Kesehatan at a national scale, serving as a reference for Indonesian healthcare stakeholders in establishing an integrated, efficient, and patient-centered digital health ecosystem.
