Abstract
PT. XYZ merupakan perusahaan yang bergerak di bidang industri Sigaret Kretek Tangan (SKT) padat karya dengan kapasitas produksi mencapai ±1.150.000 batang per hari. Berdasarkan data historis periode 2024 hingga Juni 2026, tercatat tren kasus cedera ringan (first aid cases) yang belum menurun secara signifikan, dengan 40% hingga 50% dari total insiden tersebut berpusat pada area pemotongan (cutting) rokok akibat pengoperasian alat manual push-cutting. Praktik keinsinyuran ini bertujuan mengidentifikasi potensi bahaya (hazard), mengevaluasi tingkat risiko operasional secara terukur, membedah akar penyebab kecelakaan kerja, serta merumuskan usulan pengendalian yang aplikatif. Metodologi yang digunakan mengintegrasikan metode Hazard Identification, Risk Assessment, and Risk Control (HIRARC) untuk penentuan level matriks risiko, yang dipadukan dengan Teori Domino Heinrich guna menganalisis tindakan tidak aman (unsafe act) dan kondisi tidak aman (unsafe condition) di lantai produksi. Data dikumpulkan melalui observasi lapangan, dokumentasi instruksi kerja, wawancara, dan kuesioner kepada jajaran manajemen, supervisor, staff, serta operator stasiun kerja terkait. Hasil analisis menunjukkan bahwa potensi bahaya yang paling dominan pada proses cutting adalah tindakan mengambil dan memegang pisau saat pemasangan/pelepasan pisau push cutter, serta pengambilan rokok yang jatuh di kolong alat, dengan risiko dominan berupa cedera sayat pada tangan dan jari. Secara kuantitatif, dari 21 item pekerjaan yang teridentifikasi pada proses cutting, hasil plot matriks risiko menunjukkan 3 item (14%) berada pada level risiko Tinggi (High), 11 item (52%) level risiko Sedang (Medium), dan 7 item (33%) level risiko Rendah (Low). Berdasarkan perhitungan Importance Index (IMP.I) dari data kuisioner 7 responden, tiga peringkat teratas adalah aktivitas mengambil pisau kanan dari kotak dan meletakkannya di rumah pisau dengan IMP.I 0,611, aktivitas mengambil pisau pada bagian ujung dengan tangan kanan dengan IMP.I 0,583, serta pengambilan rokok yang jatuh di bawah alat push cutter dengan IMP.I 0,583. Analisis dengan Metode Domino mengungkapkan bahwa faktor penyebab dominan bersumber dari lemahnya kontrol keselamatan (lack of control) dan sosialisasi Standard Operational Procedure (SOP) yang belum maksimal, ketidakdisiplinan penggunaan Alat Pelindung Diri (APD), serta kelalaian pekerja saat bekerja. Berdasarkan temuan tersebut, dirumuskan usulan pengendalian risiko yang mencakup rekayasa teknik berupa pemasangan safety guard akrilik pada alat push cutter, pengendalian administratif berupa penyegaran instruksi kerja serta penjadwalan pemasangan/pelepasan pisau yang diawasi supervisor dan group leader, pemasangan rambu keselamatan (safety sign), dan penggunaan alat pelindung diri berupa sarung tangan anti sayat. Rekomendasi ini diharapkan dapat menekan angka kecelakaan kerja pada proses cutting serta mendukung terciptanya budaya K3 yang berkelanjutan di PT. XYZ.

PT. XYZ is a company operating in the labor-intensive hand-rolled cigarette (SKT) industry, with a daily production capacity of approximately 1,150,000 sticks. Based on historical data from 2024 to June 2026, a persistent trend of minor injuries (first aid cases) has been recorded, with 40% to 50% of these incidents concentrated in the cigarette cutting area due to the manual operation of push-cutting tools. This engineering practice aimed to identify workplace hazards, assess operational risk levels, examine the root causes of accidents, and formulate applicable risk control measures. The methodology combined the Hazard Identification, Risk Assessment, and Risk Control (HIRARC) method to determine risk matrix levels with Heinrich's Domino Theory to analyze unsafe acts and unsafe conditions on the production floor. Data were collected through direct field observation, work instruction documentation, interviews, and questionnaires distributed to management, supervisors, staff, and workstation operators. The results show that the most dominant hazards in the cutting process are handling the blade during installation/removal of the push cutter and retrieving cigarettes that fall beneath the machine, with hand and finger cuts as the dominant risks. Quantitatively, of the 21 identified work items in the cutting process, the risk matrix plotting showed 3 items (14%) at a High risk level, 11 items (52%) at a Medium risk level, and 7 items (33%) at a Low risk level. Based on the Importance Index (IMP.I) calculated from questionnaire data of 7 respondents, the top three ranked items were taking the right-hand blade from the box and placing it in the blade housing (IMP.I 0.611), taking the blade at its tip with the right hand (IMP.I 0.583), and retrieving cigarettes that fall beneath the push cutter machine (IMP.I 0.583). Domino Method analysis revealed that the dominant causal factors stem from inadequate safety control (lack of control) and insufficient socialization of the Standard Operational Procedure (SOP), workers’ non-compliance with personal protective equipment (PPE), and momentary lapses in worker attention. Based on these findings, risk control recommendations were formulated, including engineering controls such as installing an acrylic safety guard on the push cutter, administrative controls such as refreshing work instructions and supervisor/group leader-supervised blade-change scheduling, safety sign installation, and personal protective equipment in the form of cut-resistant gloves. These recommendations are expected to reduce workplace accidents in the cutting process and support a sustainable safety culture at PT. XYZ.
