Abstract
PLTU PT PJB UP Gresik merupakan salah satu pembangkit listrik
tenaga uap yang berada di area Jawa dan Bali. Pada steam drum unit 4
PJB Gresik ini berfungsi untuk menghasilkan uap, maka dari itu kualitas
air harus dijaga supaya uap yang dihasilkan berkualitas baik. Penerapan
metode hazard and operability analysis (HAZOP) pada tugas akhir ini
bertujuan untuk menganalisis bahaya yang terjadi pada steam drum unit
4 PLTU PJB Gresik serta menerapkan manajemen risiko untuk
mengetahui risiko dalam menghadapi bahaya yang terjadi. Setelah
dilakukan analisis, risiko bahaya tinggi terjadi pada plant steam drum. Hal
ini dikarenakan hasil risk matrix pada steam drum menunjukkan risiko
tinggi pada kondisi Ekstrem dengan nilai 16. Equipment pada steam drum
yang memiliki nilai risiko tinggi adalah drum level bernilai 4. Penyebab
drum level pada steam drum mengalami kegagalan dikarenakan kualitas
air yang masuk tidak sesuai (air laut masuk), overheating sehingga terjadi
perubahan temperature cukup tinggi. Karena tingkat bahaya dari steam
drum dapat memicu terjadinya ledakan sehingga diterapkan emergency
response plan untuk meminimalisir dampak yang terjadi.

PT PJB UP Gresik power plant is one of the steam power plant in
the area of Java and Bali. Steam drum unit 4 PJB Gresik is serves
to generate steam, and therefore the water quality must be
maintained in order to produce the good quality of vapor.
Application of the method hazard and operability analysis
(HAZOP) in this thesis is aims to analyze the dangers that occur in
the steam drum unit 4 PJB Gresik power plant and apply the risk
management to find out what are the risks in the face of the
dangers. After doing the analysis, the high hazard’s risk is likely to
occur in the plant of steam drum. This is can occurs because the
risk matrix results in the plant of steam drum is indicates the high
risk in extreme conditions with a value of 16. The instruments in
the plant of steam drum that has a high risk score is drum level
that each of the instruments has a value of 4. Cause of drum level
on steam drum failure are quality of incoming water (sea water)
and overheating. Because the highest danger in the steam drum
can cause a leakage and trigger an explosion, then given the
emergency response plan to minimize the impacts.
