Abstract
Manajemen risiko di proyek konstruksi bertujuan
untuk meminimalkan dampak terhadap keselamatan,
kesehatan, dan lingkungan kerja. dalam upaya menciptakan
lingkungan yang aman, sehat, efisien dan produktif
sebagaimana ketentuan di OHSAS 18001. Potensi risiko
mungkin juga terjadi pada proyek konstruksi Bendungan
Tugu. Proyek tersebut merupakan icon terbaru kota
Trenggalek yang sangat beresiko dalam hal kecelakaan kerja.
Penelitian tugas akhir ini bertujuan untuk mengetahui risiko –
risiko yang paling dominan, dan mengetahui faktor – faktor
dari risiko – risiko yang paling dominan tersebut.
Dalam penelitian ini digunakan 2 metode yaitu
metode Failure Mode and Effect Analysis (FMEA) dan
metode domino. Metode FMEA untuk mengetahui risiko
yang paling dominan dan metode domino untuk mengetahui
dan menganalisa penyebab resiko tersebut. 3 hasil RPN
tertinggi failure mode bendungan tugu adalah : Sling dari bore
pile putus, Tertimpa stock material batu,Tergores ujung besi cor, Tertimpa benda berat,kena benda tajam,kena ledakan. 3
hasil Maximum RPN kemudian dianalisis di metode domino:
Pertama sling dari bore pile putus masing-masing adalah lack
of control (terbatasnya pengawasan & safety dilapangan),
basic couse (kurangnya pengetahuan dan pemakaian/keausan
alat), immediate couse (kondisi lapangan berbahaya) yang
menimbulkan Loss (alami luka ringan/berat dan merusak
properti alat). Yang Kedua untuk tertimpa stock material batu,
tergores ujung besi cor masing-masing adalah Lack of control
(terbatasnya pengawasan dan kurangnya pelatihan K3) yang
sebabkan Incident (Tertimpa stock material batu,tergores
ujung besi cor) menimbulkan Loss (luka berat/ringan).
Tertimpa benda berat, benda tajam, ledakan faktor dasar dari
failure modenya adalah kurang pengetahuan dan kurang
terampil (Basic couse), menimbulkan kerugian luka berat
yaitu kematian dan kehilangan waktu produksi.

Risk management in construction project aims to
minimize the impact on safety, health and working
environment. in an effort to create an environment that is
safe, healthy, efficient and productive as stipulated in OHSAS
1800. A potential risk would also be possible happen in
Bendungan Tugu. The project is that becomes a new
landmark in trenggalek highly risky in terms of workplace
accidents. This research aims to determine the risk - the risk
of the most dominant, and identify factors - factors of risk -
the risk of the most dominant.
This study aplied two methods, they are the Failure
Mode and Effect Analysis (FMEA) and dominoes. FMEA
method is to determine the risk of the most dominant
whiledomino method to identify and analyze the causes of
such risks.The result of RPN show that 3 risk events,is: Sling
of bore pile breaking, Crushed stone material stocks,
Scratched end of cast iron, Crushed heavy objects, contact
with sharp object, hit explosion. These RPN are analyzed in a
domino. Method: First dominan event slings of bore pile
breaking each is lack of control (limited supervision and
safety field), basic couse (lack of knowledge and use / wear of
the tool), immediate couse (field conditions dangerous) which cause Loss (natural minor injuries / weight and damaging
property tools). The second Crushed stone for stock material,
scraped the tip of cast iron, respectively Lack of control (lack
of supervision and lack of training K3) which caused Incident
(Crushed stone material stocks, scraped the tip of cast iron)
pose Loss (severe injuries / minor). Crushed by heavy objects,
sharp objects, explosions basic factors of failure modes is less
knowledge and less skilled (Basic couse), causing severe
injuries that death losses and lost production time.
