Abstract
Dalam dunia perikanan diperlukan usaha untuk menjaga kesegaran ikan, cold storage adalah mesin pendingin yang difasilitasi oleh peralatan pendingin seperti kompresor, kondensor dan evaporator, dan memiliki fungsi sebagai penyimpanan bahan atau produk seperti produk pertanian dan produk perikanan, untuk menjaga bahan atau produk dalam kondisi utuh dan memperlambat kinerja bakteri sehingga pembusukan bisa menjadi terelakkan, cold storage dalam pembekuan ikan terkadang memiliki masalah. Masalah umum terkadang terjadi pada kegagalan sistem atau pengelolaan. Cold storage pada PT.X mengalami kerusakan akibat membusuknya penyimpanan perikanan dalam penyimpanan cold storage, penyebab kerusakan tidak diketahui. Untuk mengetahui penyebab kerusakan diperlukan survey investigasi forensik dalam pengambilan data, dan analisa penelitian ini akan menggunakan Apollo Root Cause Analisis (ARCA) pada perangkat lunak RealityCharting. Proses investigasi dan analisa charting digunakan untuk mengetahui penyebab kerusakan pada cold storage yang terhadi pada PT.X, yang pada akhirnya kerusakan terdapat pada sistem pendingin terjadi kerusakan separator, penyumbatan oli dan refrigran, evaporator kotor dan kesalahan penyusunan rak, solusi akan diberikan dalam bentuk saran dan prosedur operasional dalam cold storage

In preserving fishery is needed to maintain freshness of fish, cold storage is a cooling machine that is facilitated by cooling equipment such as compressors, condensers and evaporators, and has a function as storage of materials or products such as agricultural products and fishery products, which keep the material or product in intact conditions to slow the performance of bacteria so that decay can be inevitable, cold storage in fish freezing sometimes has problems. Common problems sometimes occur in system or management failures. Cold storage on PT.X suffered damage resulting in loss in fishery production, cause cold storage damage not known which cause of the damage, To know cause of damage of forensic investigation in need to conduct survey in taking data and research will use Apollo Root Cause Analysis (ARCA) by using RealityCharting software to find the cause of damage to the cold storage, which in the end damage is found in the compressor cooling system, evaporator and mistake in the preparation of fish, and solution problems will be provided in the form of operational procedures in cold storage
