Abstract

ICU merupakan salah satu bagian penting dalam rumah sakit. Di ICU pasien akan dicek kondisi serta prognosisnya secara intensif. Prognosis dilakukan untuk memutuskan pengobatan apa yang terbaik bagi pasien sehingga ICU dapat memberikan pelayanan yang lebih baik. Dalam menentukan prognosis pasien, saat ini ahli medis banyak menggunakan bantuan sistem scoring yang memiliki beberapa kekurangan seperti parameter yang diuji banyak, membutuhkan waktu lama, dan tidak merefleksikan kondisi pasien kedepannya. Padahal penilaian prognosis dapat juga dianalisis dengan memanfaatkan sinyal elektrokardiogram yang didapatkan dari bedside monitor ICU. Untuk melakukan analisis prognosis pasien, sinyal elektrokardiogram dikonversi menjadi sinyal Heart Rate Variability (HRV) dengan melakukan perhitungan R-R intervalnya. Kemudian sinyal HRV dilakukan ekstraksi fitur menggunakan Higher-Order Moments Detrended Fluctuation Analysis untuk melihat struktur non-linear dan melihat volatility pada sinyal HRV dengan melakukan perhitungan nilai skewness dan kurtosis. Output dari ekstraksi fitur adalah berupa nilai fungsi fluktuasi dan local scaling eksponent berdasarkan skewness dan kurtosis pada pasien prognosis baik dan buruk. Nilai tersebut kemudian digunakan sebagai input untuk klasifikasi menggunakan Artificial Neural Network. Output dari klasifikasi tersebut adalah klasifikasi prognosis pasien ICU. Hasil klasifikasi ekstraksi fitur HOMDFA menghasilkan model terbaik pada model dengan jumlah scale 5, hidden node 8, learning rate 1, momentum 0,1 dan 0,2. Model tersebut menghasilkan nilai accuracy sebesar 74.19%, sensitifity 0.742, specificity 0.39, dan AUC sebesar 0.681. ============== ICU is one important part of the hospital. In the ICU, the patient will be checked for the condition and the prognosis intensively. The prognosis is performed to decide what treatment is best for the patient so that the ICU can provide better services. In determining the patient's prognosis, many medical experts now use scoring systems that have some disadvantages such as many tested parameters, take a long time, and do not reflect the patient's condition going forward. Though the prognosis assessment can also be analyzed by utilizing electrocardiogram signals obtained from bedside ICU monitors. To perform a patient prognosis analysis, the electrocardiogram signal is converted into a Heart Rate Variability (HRV) signal by performing R-R interval. Then HRV signals feature extracted using Higher-Order Moments Detrended Fluctuation Analysis to see non-linear structures and volatility on HRV signals by calculating skewness and kurtosis values. The output of feature extraction is the value of fluctuation function and local scaling exponent based on skewness and kurtosis in patients with good and bad prognosis. The value is then used as an input for classification using the Artificial Neural Network. The output of the classification is the classification of prognosis of ICU patients. the best model of HOMDFA feature extraction classification are model with scale 5, hidden node 8, learning rate 1, momentum 0,1 and 0,2. The model produces accuracy values of 74.19%, sensitivity 0.742, specificity 0.39, and AUC of 0.681.
