Abstract
Sertifikat alat safety merupakan salah satu dokumen yang wajib dimiliki oleh kapal ketika berlayar. Sertifikat tersebut menjadi jaminan bahwa alat-alat safety yang dibawa oleh kapal dapat digunakan sesuai fungsinya masing-masing. Berdasarkan data dari KNKT, terdapat beberapa kejadian kecelakaan kapal dengan keadaan rakit penolong (inflatable life raft) tidak mengembang ketika dilemparkan ke laut, sehingga jumlah korban jiwa semakin bertambah. PT. Surya Segara Safety Marine (PT. SSSM) merupakan perusahaan yang bergerak pada inspeksi, pengujian dan perawatan alat safety kapal. Perusahaan ini bertanggung jawab terhadap penerbitan sertifikat lolos uji yang berkualitas. Jika didapatkan ketidak sesuaian sertifikat dengan fakta investigasi, maka perusahaan akan mendapatkan hukuman dari Syahbandar. Hukuman terringan berupa peringatan hingga yang terberat berupa penghentian sementara. Oleh karena itu, identifikasi dan pengelolaan potensi kegagalan proses penerbitan sertifikat menjadi hal yang penting dilakukan untuk memberikan jaminan kualitas dari sertifikat yang dikeluarkan. Salah satu metode untuk identifikasi dan pengelolaan potensi kegagalan adalah FMECA. Pada penelitian ini, FMECA diterapkan dengan melakukan identifikasi potential failure mode dan identifikasi cause of potential failure. Sehingga potensi kegagalan dan penyebab potensi kegagalan pada perusahaan, dapat dikelola secara optimal. Setelah dilakukan identifikasi, berhasil didapatkan 493 potensi failure mode dan 1099 potential cause of failure sebagai penyebab yang menimbulkan. Setelah dilakukan penilaian criticality, didapatkan nilai RPN tertinggi yaitu 75 dengan 3 potential cause of failure yang berbeda. Kemudian dipilih empat nilai RPN tertinggi untuk dilakukan tindakan mitigasi. Tindakan mitigasi ini memberikan efek pada penurunan nilai occurrence serta sebagian kecil dari nilai severity pada potential failure mode. Empat nilai RPN tertinggi tersebut berjumlah 40 potential failure mode atau mewakili 3,9% dari potential cause of failure. Jika dilakukan tindakan mitigasi, maka mampu memberikan efek sebesar 9,1% terhadap penurunan nilai RPN pada potential failure mode.

Safety Equipment Certificate is one of the documents that ship must own when sailing. The certificate guarantees that the safety equipment carried by the ship can be used as respective functions. Based on data from KNKT, there are several incidents of shipwreck with the state of the inflatable life raft does not expand when thrown into the sea, causing the increase of fatalities. PT. Surya Segara Safety Marine (PT SSSM) is a company engaged in the field of inspection, testing and maintenance of ship safety equipment. The company is responsible for the issuance of qualified test certificates. If there is any mismatch between the certificate and the real investigated condition, then the company will be punished, from getting warned by Syahbandar as the lightest punishment until a temporary suspension as the worst punishment. Therefore, the identification and management of the potential failure of the certificates issuance become important to be generated in order to provide quality assurance of issued certificates. A method to identify and to manage potential failures is FMECA. In this research, FMECA is conducted by identifying potential failure mode and cause of potential failure so that the potential failure and the potential cause of failure in the company can be optimally managed. After identification process, it is obtained 493 potential failure modes and 1099 potential causes of failure. After the criticality has been assessed, it is obtained that 75 is the highest value of with 3 different potential causes of failure. Then the four highest RPN values are selected for mitigation action. This mitigation action affects the decrease of occurrence value and minority of the severity value in Potential Failure Mode. The four highest RPN values are as much as 40 potential failure modes, representing 3.9% of the potential cause of failure. If mitigation is implemented, it is able to decrease 9,1% of RPN value in potential failure mode.
