Abstract
Proses perbaikan kualitas suatu produk merupakan hal yang sangatlah penting dalam suatu proses pembuatan produk dalam industri manufaktur. Beberapa metode yang seringkali digunakan untuk proses perbaikan kualitas yaitu Design of Experiment (DOE), Statistical Process Control (SPC) dan Failure Mode and Effect Analysis (FMEA). Penelitian ini bertujuan untuk mengembangkan tool perbaikan kualitas yang baru dengan mengintegrasikan House of Quality (HOQ), Failure Mode and Effect Analysis (FMEA) dan Root Cause Analysis (5 why’s) yang nantinya akan menawarkan cara baru untuk proses perbaikan kualitas produk. pengembangan tool untuk perbaikan kualitas ini mengadopsi matriks yang dikembangkan oleh peneliti sebelumnya pada House of Risk (HOR). Modifikasai HOQ, FMEA dan RCA pada penelitian ini dilakukan untuk mengakomodasikan proses perbaikan kualitas dengan cara mengidentifikasi defect, akar penyebab defect dan menentukan usulan perbaikan untuk menanggulangi akar defect. Usulan tool baru untuk perbaikan kualitas ini terdiri dari dua matriks dimana Pada matriks pertama bertujuan untuk mengidentifikasi defect, akar penyebab defect dan menentukan prioritas akar penyebab defect. Selanjutnya pada matriks yang kedua akan mencari dan juga memilih alternatif tindakan untuk perbaikan kualitas.

The process of improving the product quality is very important in a process of manufacturing products in the manufacturing industry. Several methods are often used for quality improvement process: Design of Experiment (DOE), Statistical Process Control (SPC) and Failure Mode and Effect Analysis (FMEA). This research aims to develop new quality improvement tools by integrating House of Quality (HOQ), Failure Mode and Effect Analysis (FMEA) and Root Cause Analysis (5 why's) which will offer new ways to improve product quality. the development of this quality improvement tool adopted a matrix developed by previous researchers on House of Risk (HOR). Modification of HOQ, FMEA, and RCA in this study was conducted to accommodate the process of quality improvement by identifying defects, root causes of defects and determining proposed improvements to tackle the root cause of defects. The proposed new tool for quality improvement consists of two matrices where in the first matrix aims to identify defects, root causes of defects and define the root cause priority defect. Next on the second matrix will search for and also choose alternative actions for quality improvement.
