Abstract

Proyek konstruksi gedung bertingkat mempunyai berbagai macam hazard (sumber bahaya) yang berpotensi menimbulkan kecelakaan kerja dan dapat menyebabkan kerugian hingga kematian pekeja. Penelitian ini menggunakan teori Swiss Cheese Model unruk menganalisis faktor penyebab kecelakaan kerja pada Proyek Grand Dharmahusada lagoon Surabaya. Pada penelitian sebelumnya, teori Swiss Cheese Model ini lebih banyak digunakan untuk menganalisis kecelakaan di industri penerbangan. Penelitian ini bertujuan untuk mendapatkan gambaran tentang sistem pertahanan dalam mencegah kecelakaan. Penelitian dilakukan dengan melakukan studi literatur, kajian terhadap hasil kuisioner dan wawancara. Dalam proses identifikasi dan melakukan analisis, penelitian ini menggunakan Risk Management Standard AS/NZS 4360. Dari hasil analisis kecelakaan kerja pada bidang proyek konstruksi yang ditinjau pada lokasi Proyek Dharmahusada Lagoon Surabaya, terdapat dua variable risiko yang memiliki risiko paling tinggi yaitu pekerjaan pengecoran di ketinggian dengan risiko terjatuh dan pekerjaan bekisting di ketinggian dengan risiko bekisting/scaffolding jatuh menimpa pekerja/fasilitas. Dari hasil analisis menggunakan teori Swiss Cheese Model, didapatkan bahwa kelalaian atau tindakan tidak aman dari para pekerja dipengaruhi oleh kebijakan organisasi.========================================================================Workplace accidents are a common occurrence in construction projects and are important things to be aware of. Until now the number of work accidents in the construction sector is still relatively high. Any workplace accidents can lead to various losses to the death of the worker. This research uses a theory that is used to analyze the factors causing work accidents, namely Swiss Cheese Model which in previous studies used to analyze aviation accidents. This study aims to get a picture of the defense system in preventing accidents in accordance with the concept of the theory.
To mitigate or eliminate hazards that may cause accidents at work, a risk management activity is required including hazard identification, hazard potential analysis, risk assessment, risk control, and monitoring and evaluation. In the process of identification and analysis, this research used Risk Management Standard AS / NZS 4360. In the next stage, risk identification was done by distributing questionnaires of preliminary survey to obtain risk variables relevant to field condition, then distributing the main survey questionnaire to find out the amount of likelihood and consequence based on the perception of the parties related to the project. After that, a risk assessment with likelihoods and consequences calculation is then obtained risk analysis matrix so it can be known the risk of occupational accidents are most dominant. Then the dominant risk was analyzed as to why the accident occurred using Swiss Cheese Model and continued with the determination of recommendation.
The results of this research, there are two risk variables that classified the highest risk. The first risk is falling from the height at casting activity. The second risk is fallen by formwork and scaffolding at formwork activity. From 4 layers Swiss Cheese Model, divided into the active failure and latent failure. The immediate failures that occur are unsafe supervision such as construction workers not paying attention to safety warnings and not being vigilant. Whereas, there are 3 layers that fall into the category of covert failure, which is the cause of unsafe actions, unsafe supervision, and organizational influence. When the defense system of each of the four layers has failed, the accident can not be avoided.
