Abstract
PT. Asahimas Flat Glass Tbk, Sidoarjo merupakan salah satu produsen kaca lembaran di Indonesia. Salah satu produk kaca yang paling cepat laku di pasaran adalah kaca glazing jenis FL 5mm. Masalah yang dihadapi perusahaan adalah tingginya defect rate pada bulan Januari 2018 yaitu sebesar 7,32%, padahal perusahaan menetapkan target defect rate sebesar 6,6%. Nilai ini juga meningkat dibandingkan defect rate bulan Desember 2017 yang hanya sebesar 6,36%. Penelitian ini bertujuan untuk mengetahui jenis cacat penyebab tingginya defect rate pada bulan Januari 2018, menganalisis akar permasalahan masing-masing jenis cacat serta memberikan usulan perbaikan menggunakan pendekatan six sigma DMAIC. Berdasarkan hasil analisis diperoleh bahwa jenis cacat dominan yang terjadi pada kaca menurut prinsip Pareto adalah bubble, chippig, dan drip. Nilai indeks kapabilitas sebesar -0,24628 dan nilai sigma sebesar 3,72σ. Jenis cacat yang dominan adalah bubble, drip, dan chipping disebabkan oleh faktor kelalaian manusia dan mesin sehingga perbaikan yang dilakukan berfokus pada manusia dan mesin. Selain itu perusahaan diharapkan dapat meningkatkan nilai sigma paling tidak 0,07σ untuk mencapai target yang ditentukan.

PT. Asahimas Flat Glass tbk, Sidoarjo is the one of flat glass factory in Indonesia. The factory always control the glass’ quality. One of the glass product that sold fast in the market is glazing glass type FL 5mm.. The problem that now faced by the factory is increasing number of defect rate in January 2018 about 7,32%, where the target of the defect rate is only 6,6% from the amount of production. This percentage is increasing from last month, December 2017 was just only about 6,36%. The purpose of the research is identification of types of defect that occurs in glass production and what cause high number defect rate in January 2018, analyzing the root of cause each type of defect and giving the alternative solutions for improval phase using six sigma method DMAIC. Based on analysis results, the types of defects that occurs using Pareto principal are bubble, chipping, and drip. Capability index value is -0,24628 and sigma level is 3,72σ. Dominant defects are bubble, drip and chipping which were caused by man and machine factor so improvement phase focused in man and machine. Then analyzing the root causes using Ishikawa diagram for each defect type. The factory should increase sigma level at least 0,07 σ for aiming the target.
