Abstract
Kegiatan operasional rumah sakit berpotensi menimbulkan dampak negatif
terhadap lingkungan. IPAL merupakan unit vital untuk mengurangi pencemaran
lingkungan terutama terhadap badan air. Data analisa limbah pada tahun 2011
sampai 2013 sering melebihi baku mutu, konsentrasi COD mencapai 108,9 mg/l,
NH3 Bebas 0,54 mg/l, Phosphat 35,8 mg/l. Penurunan kinerja IPAL selain
dipengaruhi karena adanya perubahan sistem yang tidak sesuai dengan desain,
kinerja operator, dan kondisi mesin, merupakan faktor pemicu terjadinya
penurunan kinerja IPAL.
Penelitian ini dilakukan untuk menganalisis potensi-potensi risiko yang
signifikan terhadap penurunan kinerja IPAL. Variable yang digunakan meliputi
aspek Sumber Daya Manusia (SDM), Mesin atau peralatan, dan Proses IPAL.
Setiap proses IPAL akan dianalisis menggunakan metode Failure Mode Effect
Analysis (FMEA) hingga ditemukan faktor risiko yang potensial. Risiko tersebut
kemudian dicari akar penyebab masalahnya menggunakan metode Fault Tree
Analysis (FTA), serta dihitung nilai probabilitas dan konsekuensinya untuk
menentukan kategori pada matrik risiko.
Hasil penelitian menunjukkan bahwa penyebab utama buruknya kualitas
efluen limbah cair disebabkan oleh faktor proses IPAL, dimana nilai MLSS, F/M
rasio, Sludge Age, dan SVI belum memenuhi kriteria proses. Selain itu Kinerja
Operator IPAL yang kurang konsisten juga menjadi penyebab penurunan kualitas
efluen IPAL. Optimasi diprioritaskan pada risiko dengan kategori Severe dan High,
yaitu menambah proses sirkulasi lumpur aktif dengan pompa berkapasitas minimal
23 liter/ menit dan melakukan recruitment pegawai untuk posisi operator IPAL.

Operational activities of the hospital a potential negative impact on the
environment. WWTP is a vital unit to reduce environmental pollution, especially of
the water body. Data analysis of wastewater in 2011 through 2013 COD
concentration reached 108.9 mg/ l, NH3-free 0.54 mg/ l, Phosphate 35.8 mg/ l.
WWTP performance degradation besides affected due to changes in the system that
are not in accordance with the design, the performance of the operator, and the
condition of the engine, the factors triggering the decline in performance of the
WWTP.
This study was conducted to analyze the potential significant risks to
performance degradation WWTP. Variable used include aspects of Human
Resources (HR), machine or equipment, and process at WWTP. Each WWTP
process will be analyzed using methods Failure Mode Effect Analysis (FMEA) to
find potential risk factors. The risks were then searched the root cause of the
problem using Fault Tree Analysis (FTA), and calculated value of the probability
and consequences to determine the category in the risk matrix.
The results showed that the main cause of the poor quality of wastewater
effluent caused by factors WWTP process, where the value of MLSS, F/M ratio,
Sludge Age, and SVI not meet the process criteria. In addition, performance of
WWTP Operator is less consistent also cause a decrease in the WWTP effluent
quality. Optimization prioritized on the risk category of Severe and High, which
adds to the activated sludge process with a circulation pump with a capacity of at
least 23 liters / minute and perform recruitment of employees for positions WWTP
operator.
