Abstract
Masih tinggi nya angka kecelakaan kerja dalam proyek konstruksi serta
rendahnya perhatian masyarakat terhadap hal ini membuat kecelakaan kerja
seaakan lumrah dalam tiap proyek dengan risiko bahaya yang tinggi, termasuk
pekerjaan dalam bidang maritim. SPM 150.000 DWT TTU Balongan merupakan
salah satu aset vital milik PT. Pertamina (Persero) yang sudah harus dilakukan
penggantian (change over) dan perbaikan (docking repair) dalam skala besar.
Dalam proyek change over single point mooring terdapat kegiatan-kegiatan yang
memiliki indikasi bahaya yang dapat berdampak pada aspek keselamatan, baik itu
pekerjaan di bawah atau di atas air. Pada penelitian ini dilakukan analisis bahaya
kecelakaan kerja pada proyek change over single point mooring untuk mengetahui
kecelakaan kerja dominan beserta penyebab, dampak, kontrol, serta mitigasi
dengan menggunakan metode Bowtie analysis. Dari penelitian ini diketahui
terdapat 2 hazard effect / consequences dengan tingkat kategori “ekstrim” yaitu
diver mengalami dekompresi, serta benturan antara SPM dengan work barge.

The high number of occupational accidents in construction projects and the low
attention of the public to this matter make work accidents like normal in every project
with high hazard risk, including work in maritime fields. SPM 150,000 DWT TTU
Balongan is one of the vital assets of PT. Pertamina (Persero) which has to be replaced
(change over) and repair (docking repair) on a large scale. In the project of change
over single point mooring there are activities that have a hazard indication that can
impact on the safety aspect, either it work under or above water. In this study, a hazard
analysis of work accidents was carried out on the change over single point mooring
project to determine the dominant workp accidents and their causes, impacts, controls,
and mitigation using the Bowtie analysis method. From this study it is known that
there are 2 hazard effects / consequences with the level of "extreme" categories,
namely diver experiencing decompression, and clash between SPM with work barge.
