Abstract

Instalasi Pengolahan Lumpur Tinja (IPLT) Keputih Surabaya mulai beroprasi pada tahun 1991 dengan kapasitas desain maksimum 400 m3 /hari. Nilai effluen IPLT Keputih pada analisis terakhir untuk parameter BOD5 sebesar 98 mg/L,COD sebesar 168 mg/L, TSS sebesar 46 mg/L, minyak lemak sebesar 16 mg/L, Nitrogen sebesar 15 mg/L dan fosfat sebesar 8 mg/L. Hasil analisis tersebut belum memenuhi baku mutu PermenLH No 68 Tahun 2016. Penelitian ini bertujuan untuk mengetahui risiko penyebab kegagalan di instalasi pengolahan lumpur tinja dengan menggunakan metode fishbone dan metode failure mode and effect analysis (FMEA). Metode fishbone digunakan untuk menentukan risiko-risiko penyebab kegagalan serta sumber kegagalan. Dari risiko tersebut dapat dihitung nilai severity, Occurence, dan detection dengan metode Failure Mode and Effect Analysis (FMEA). Dari nilai yang telah ada dapat ditemukan prioritas penanganan dan upaya perbaikan dengan cara mengalikan hasil nilai severity, Occurence, dan detection, metode ini dikenal dengan sebutan metode Risk Priority Number (RPN). Berdasarkan hasil analisis menggunakan metode fishbone didapatkan 6 faktor penyebab kegagalan effluen tidak memenuhi baku mutu. Dari analisis lanjutan menggunakan metode FMEA melalui hasil perangkingan didapatkan 3 peringkat teratas yang menjadi prioritas perbaikan dengan nilai RPN 100 yaitu kemampuan unit meremoval beban pencemar dengan nilai SOD sebesar 5-4-5, durasi penggantian media dengan nilai SOD sebesar 5-5-4 dan pengimplementasian standar operasional prosedur dengan nilai SOD sebesar 4-5-5.

This study aims to determine the cause of the failure risk of the treatment plant sludge using fishbone method and the method of failure mode and effect analysis (FMEA). Fishbone method used to determine the cause of the failure risks as well as the source of the failure. The risk can be calculated from the value of severity, Occurrence, and detection by the method of Failure Mode and Effect Analysis (FMEA). Of the existing value can be found priority in handling and remedial efforts by multiplying the result of the value of severity, Occurrence, and detection, this method is known as a method Risk Priority Number (RPN). Based on the analysis using method fishbone showed 6 factors causing the failure effluent does not meet quality standards. From further analysis using FMEA method of ranking the results obtained through the 3 top ranked priority improvements to the value of the RPN 100, pollutant load capability meremoval units with a value of SOD at 5-4-5, the duration of the replacement of media with a value of 5-5-4 and SOD implementation of standard operating procedures with the value of SOD at 4-5-5.
