Abstract
Dalam rangka revitalisasi industri pupuk dan peningkatan daya saing
global, PT Pupuk Kaltim membangun pabrik Kaltim-5 yang resmi beroperasi
November 2015. Pabrik Kaltim-5 diharapkan dapat menunjang produksi urea
untuk penjualan global serta urea bersubsidi untuk meningkatkan ketahanan
pangan nasional. Sebagai salah satu aktivitas baru di PT Pupuk Kaltim, sesuai
dengan kebijakan manajemen risiko perusahaan, perlu diidentifikasi risiko-risiko
yang melekat pada unit urea Kaltim-5 agar rate produksi unit urea Kaltim-5 tidak
terhambat. Untuk proses produksi, dapat dilakukan analisa risiko dengan metode
FMEA. Namun, ambiguitas dan subjektifitas penilaian pada metode FMEA cukup
tinggi. Dapat digunakan Fuzzy-logic untuk mengurangi ambiguitas dan
subjektifitas dari penilaian metode FMEA.,
Penelitian ini dilakukan dengan menggunakan framework manajemen
risiko ISO 31000. Di awali dengan brainstorming dengan pihak expert untuk
mengidentifikasi risiko pada unit urea Kaltim-5. Setelah risiko diidentifikasi,
dilakukan analisa risiko dengan FMEA untuk mendapatkan nilai severity,
occurence dan detection yang akan di-fuzzifikasi dan diberikan bobot untuk
mendapatkan nilai Fuzzy Risk Priority Number (FRPN). Selanjutnya diberikan
mitigasi risiko untuk risiko yang memiliki nilai FRPN tinggi dan berada pada
kondisi extreme risk sesuai peta risiko

In order to fertilizer industry revitalization and to increase global
competitiveness, PT Pupuk Kaltim build for Kaltim-5, which was officially
opened November 2015. Kaltim-5 is expected to support the production of urea
for global sales and subsidized urea to improve national food security. As one of
the new activity at PT Pupuk Kaltim, in accordance with the risk management
policies of the company, need to be identified risks attached to the urea unit
Kaltim-5 so the production rate of urea unit Kaltim-5 is not impeded. For the
production process, to do risk analysis FMEA method can be used. However,
ambiguity and subjectivity in FMEA method assesment is quite high. Fuzzy-logic
can be used to reduce ambiguity and subjectivity of FMEA method assesment.
The research was done by using the risk management framework of ISO
31000. In starting with a brainstorming with the expert to identify risks to the urea
unit Kaltim-5. Once risk is identified, FMEA is used to get the value of severity,
occurence and detection that will be do fuzzification and assigned weights to get
the value of Fuzzy Risk Priority Number (FRPN). Furthermore, given mitigating
risk to risk that having high value FRPN and at extreme risk conditions
appropriate risk maps
