Abstract

Kerusakan bolt dari brake assembly dan impeller dari brake cooling fan assembly pada pesawat ditemukan karena adanya indikasi overheat pada kokpit sehingga perlu dilakukan transit checking sebelum pesawat kembali terbang. Kerusakan ini menunda keberangkatan selanjutnya dari pesawat dan berbahaya bagi keselamatan, sehingga perlu dilakukan failure analysis. Failure analysis dilakukan pertama-tama dengan melakukan observasi awal akibat adanya indikasi overheat, dilanjutkan dengan studi literatur, dan pengambilan data mengenai komponen dan kerusakannya. Material komponen diuji menggunakan spectrometer thermo ARL dan pola patahannya diamati secara makroskopik menggunakan kamera digital dan stereo microscope. Dari penelitian pada komponen yang rusak, dihasilkan kesimpulan bahwa semua blade dari impeller mengalami brittle fracture akibat impact dengan shroud grid support. Patahnya blade 1 menyebabkan impeller tidak seimbang sehingga blade lainnya patah. Bolt 1 mengalami low cycle fatigue fracture akibat beban siklik yang lebih kecil dari yield strength material, sedangkan bolt 2 dan 3 mengalami overload brittle fracture akibat beban yang lebih besar dari tensile strength material. Patahnya bolt 1 menyebabkan bolt lainnya mendapat beban yang lebih besar, menyebabkan imbalance, dan mempercepat patahnya bolt 2 dan 3. Kerusakan pada blade 1 dan bolt 1 dapat disebabkan oleh sistem yang tidak seimbang (imbalance)

Failures of brake cooling fan assembly impeller and brake assembly bolt were found in a plane because of the cockpit overheat indication, requiring a transit checking prior to the next departure. The failures were hazardous and delayed the next flights of the mentioned plane so it is necessary to conduct a failure analysis regarding those failures. The first step of the analysis was an observation of the indicated parts: the brake assembly and the brake cooling fan assembly, followed by a literature research, and further data collections – ages, locations, and functions – of the failing parts. The failing parts materials were tested using Spectrometer Thermo ARL and the fracture surfaces were observed using digital camera and stereo microscope. The failure analysis found that all the impeller blades suffered brittle fractures because of several impacts with the component behind, shroud grid support. The 1st blade fracture decreased the impeller stability, and later caused the fracture of other blades. Surface fracture profiles indicated low cycle fatigue fracture of the 1st bolt caused by a cyclic load smaller than the material yield strength whereas the 2nd and the 3rd bolt surface fracture profiles indicated overload brittle fracture, having undergo a load greater than the ultimate tensile strength. Imbalance caused by the 1st bolt failure hastened the failure of the two other bolts. Failures of the 1st blade and the 1st bolt could be the result of system imbalance
