Abstract
Berkembangnya ekosistem bisnis dan teknologi menuntut industri untuk senantiasa melakukan penyesuaian. Perubahan rantai pasok menjadi salah satu penyesuaian yang harus dilakukan. Proses perubahan ini memungkinkan terjadinya risiko yang dapat merugikan perusahaan. Penerapan manajemen risiko yang tepat dapat meminimalisir dampak terjadinya suatu risiko. Sebagai salah satu perusahaan jasa vending machine di Indonesia, PT Solusi Layanan Mandiri (Lunari) memiliki rantai pasok yang panjang, meliputi proses produksi, perakitan, pengiriman dan pelayanan pelanggan. Proses tersebut harus berjalan baik, sehingga menimbulkan kepuasan bagi konsumen dan para mitra usaha. Penelitian ini bertujuan untuk mengidentifikasi risiko-risiko potensial beserta penyebabnya hingga didapatkan rencana aksi mitigasi untuk menghindari atau meminimalisir risiko yang timbul pada perusahaan layanan vending machine sejenis di PT Lunari. Desain penelitian ini menggunakan metode House of Risk (HOR) yang terbagi dalam dua tahap, tahap satu digunakan untuk identifikasi risiko dan penyebab risiko yang memiliki luaran berupa agen risiko prioritas setelah di analisis Pareto lalu HOR tahap dua memiliki luaran berupa rencana aksi mitigasi dari penyebab risiko prioritas. Teknik pengambilan data diperoleh berdasarkan hasil wawancara dan penilaian menggunakan kuisioner online dan offline oleh para expert. Hasil penelitian pada HOR 1 telah teridentifikasi sebanyak 94 risiko dan 94 penyebab risiko serta terpilih 35 agen risiko prioritas. Dari hasil HOR 2 telah teridentifikasi 43 aksi mitigasi prioritas dengan usulan aksi mitigasi prioritas tertingi yaitu melakukan pengawasan ketat dalam sistem.

The development of business and technology ecosystems demands industry to always make adjustments. Supply chain changes become one of the adjustments to be made. This process of change allows risks that may harm the company. Proper implementation of risk management can minimize the impact of risk . As one of the company's vending machine services in Indonesia, PT. Solusi Layanan Mandiri (Lunari) has a long supply chain, including the process of production, assembling, shipping and customer service. The process should go well, giving satisfaction to consumers and business partners. This research aims to identify potential risks and causes until the proposed mitigation action plan to avoid or minimize the risks that arise at similar vending machine service companies in the PT Lunari. The design of this research uses the House of Risk (HOR) method which is divided into two phases, phase one is used for risk identification and risk causes that have an external priority risk agent after the Pareto analysis then HOR phase two proposed mitigation action plan from the cause of priority risk. Data retrieval techniques are obtained based on the results of interviews and assessments using online and offline questionnaire by expert. Research results on HOR 1 has been identified as much as 94 risks and 94 causes of risk as well as selected 35 priority risk agents. From the results of HOR 2 has been identified 43 priority mitigation action with proposed high priority mitigation action is Strict supervision in the system.
