Abstract
Salah satu komponen dari sebuah struktur bangunan lepas pantai yang memegang peranan paling penting dalam distribusi hasil olahan hidrokarbon adalah pipa bawah laut. Oleh karena itu penelitian ini bertujuan untuk menghasilkan Safety Case Document guna menyajikan tindakan pencegahan dan penanganan sebagai bentuk manajemen risiko kerusakan pada pipa bawah laut. Pipa bawah laut yang menjadi objek pada penelitian ini berlokasai di bagian Timur Laut Pulau Jawa, memiliki panjang sekitar 1 km. Metodologi yang digunakan pada penelitian ini diantara lain adalah metode Fishbone Diagram untuk mengidentifikasi faktor penyebab dari kerusakan pipa bawah laut. Setelah itu dilanjutkan dengan analisa mode kegagalan dan dampak kegagalan, serta perhitungan Fuzzy Risk Priority Number dengan metode Fuzzy FMEA. Setelah melewati tahapan tersebut maka dilanjutkan dengan metode HAZOP guna menghasilkan penanganan risiko yang tepat. Hasil yang telah didapatkan pada penelitian Tugas Akhir ini adalah kerangka Safety Case Document yang terdiri dari Tujuan dan Deskripsi Fasilitas, Formal Safety Assessment, dan Safety Management System.

Offshore pipeline is an indispensable component that plays a huge role in the distribution of hydrocarbon products. Thus, it is necessary to prevent any kind of incidents that may bring harm to the offshore pipeline system. Therefore, this final project is intended to develop the structure of Safety Case Document which provides preventions and mitigations actions and serves as risk management platform for offshore pipeline. Offshore pipeline that was specifically being used as a data in this project is located at Northeast of Java Sea and possesses 1 km of length, and also a subject of pipeline leakage. The methods that was being involved in this project are as follows; Fishbone Diagram to identify the cause of the failure of the pipeline, Fuzzy FMEA to determine the Risk Priority Number in order to find out the priority of each failure cause, also HAZOP to provide the necessary prevention and mitigation actions in accordance to each cause and subsequently being structured as a whole Safety Case Document with appropriate Safety Management System which also the product of this final project. The structure of Safety Case Document consists of Objective and Facility Description, Formal Safety Assessment, and Safety Management System.
