Abstract

Munculnya pandemi Covid-19 menyebabkan beberapa perubahan dalam pelayanan kesehatan yang dilakukan oleh rumah sakit. Operasional penanganan medis selama masa pandemic Covid-19 menyebabkan meningkatnya limbah yang dihasilkan dari kegiatan medis dan kegiatan pelayanan operasional, menyebabkan Standar Operasional Prosedur (SOP) harus disesuaikan dengan peraturan baru untuk mencegah kontaminasi silang selama pandemi ini. Penyesuaian ini mencakup kolaborasi proses manajemen operasi dan manajemen dokumen. Proses-proses dalam manajemen operasi dan manajemen dokumen ini berinteraksi di seluruh alur sistem, di mana tidak bisa digambarkan dengan proses model tunggal dengan Discrete Event Simulation. Meningkatnya sampah yang dihasilkan dari aktivitas operasional dan penyesuaian SOP berpotensi menyebabkan peningkatan biaya pengolahan sampah medis akibat pelayanan operasional Covid-19 dan menyebabkan waktu pelayanan menjadi lebih lama. Berdasarkan penyesuaian tersebut, penelitian ini bertujuan untuk mencari nilai optimal resource level berdasarkan biaya operasional dan waktu pelayanan dari penanganan limbah medis yang dikembangkan dengan hybrid Discrete Event Simulation – Agent-Based Modeling (DES-ABM) untuk menangkap kejadian secara melalui simulasi. Untuk mencari nilai optimum, teknik optimasi seperti Genetic Algorithm dan Goal Programming juga digunakan. Optimasi dari penelitian ini menghasilkan 17 alternatif tingkat sumber daya dari total 100 generasi dan 20 titik desain awal. Titik desain terbaik yang ditemukan dapat mengurangi waktu tunggu sebesar 237,63 menit, mengurangi waktu penyelesaian sebesar 353,31 menit, dan mengurangi biaya sebesar Rp93.149 dari tingkat sumber daya awal yang digunakan oleh rumah sakit.

The emergence of the Covid-19 pandemic has caused several changes in healthcare services carried out by hospitals. Covid-19 has caused increasing waste generated from medical activities and operational service activities, causing the Standard Operating Procedure (SOP) should be adjusted to new regulations to prevent cross-contamination during this pandemic. This adjustment includes collaboration of operation management process and document management. These processes in operation management and document management interact throughout the system, where single model process represented by Discrete Event Simulation fails to illustrate the interaction. The increasing number of wastes generated and adjustments in SOP could potentially causes increases costs for processing medical waste caused by Covid-19 operational services and causes longer service time. Based on these adjustments, this study aims to find the optimum value of resource level based on operational costs and service time from the medical waste handling developed with hybrid Discrete Event Simulation – Agent-Based Modelling (DES-ABM) to capture real-time events. To find optimum value, optimization techniques such as Genetic Algorithm and Goal Programming are also used. Optimization from this research results in 17 alternatives of resource level from a total of 100 generations and 20 initial design point. The best design point found could reduce the waiting time by 237.63 minutes, reduce completion time by 353.31 minutes, and reduce cost IDR93,149 from the initial resource level used by the hospital.
