Abstract

Penyembuhan luka merupakan proses pengembalian jaringan yang rusak ke keadaan normal melalui tahapan antara lain inflamasi, proliferasi, dan maturasi. Beberapa penelitian membuktikan bahwa penyembuhan luka dapat dipercepat dengan aplikasi topikal menggunakan herbal bersifat antioksidan dan dressing biopolimer antara lain gelatin. Tujuan penelitian ini untuk mengetahui pengaruh gelatin kulit ikan patin (KIP) dan ekstrak daun binahong (EDB) dalam proses percepatan penyembuhan luka. Ekstrak gelatin KIP diperoleh dengan metode perendaman asam (CH3COOH)–basa (NaOH). EDB diperoleh dengan metode sokhletasi menggunakan pelarut ethanol 96% dan diperoleh 1 mg pasta EDB setiap 71,9 mg daun segar. Selanjutnya dilakukan pembuatan film gelatin (FG), dimana 1 mg FG mengandung 0,89 mg gelatin kering. Film campuran gelatin dengan EDB (FGB) dibuat dengan rasio 1:1. EDB, FG, dan FGB diuji aktivitas antioksidannya menggunakan DPPH. Mikrostruktur FG dan FGB diamati menggunakan Scanning Electron Microscope. Perlakuan penyembuhan luka terdiri dari kontrol tanpa perlakuan (KN), kontrol povidone iodine (KP), tetes ekstrak daun binahong (TB), dressing FG, dan dressing FGB, selama 14 hari dengan pengamatan 3x (H2; H5; H14). Kulit sehat pada hari awal pelukaan (H0) dan kulit pada H14 dibuat preparat histologi dengan pewarnaan Masson’s Trichrome kemudian dianalisis nilai semikuantitatif histologis. Pengaruh perlakuan terhadap ukuran diameter (mm) luka di akhir perlakuan dianalisis dengan ANOVA one way. Aktivitas antioksidan (IC50) pada EDB; FG; FGB berturut-turut adalah 376,89 µg/ml; tidak terdeteksi; 14,263 ppm. Gambaran SEM mikrostruktur FG menunjukkan permukaan halus sedangkan FGB menunjukkan permukaan yang tidak rata (terdapat gelembung dan cekungan). Perlakuan FG dan FGB berpengaruh secara signifikan terhadap percepatan penyembuhan luka dibandingkan tanpa perlakuan. Perlakuan aplikasi film gelatin KIP dan ekstrak binahong pada luka dapat meningkatkan laju kesembuhan terutama pada fase inflamasi dan maturasi. Serta menunjukkan pengaruhnya pada ciri histologis saat fase maturasi (pada area dermis teramati kolagen berorientasi horizontal, berpola gelendong, adanya folikel rambut, dan jaringan epitel di epidermis yang lebih tebal).

Wound healing is a process of damaged tissue to a normal state through stages including inflammation, proliferation, and maturation. Several studies have shown that wound healing can be accelerated by topical application using antioxidant herbs and biopolymer dressings including gelatin. The purpose of this study was to determine the effect of catfish skin gelatin (KIP) and binahong leaf extract (EDB) in the process of accelerating wound healing. KIP gelatin extract was obtained by soaking in acid (CH3COOH) - base (NaOH). EDB was obtained by soxhletation method using 96% ethanol as solvent and 1 mg of EDB paste was obtained for every 71.9 mg of fresh leaves. Furthermore, gelatin film (FG) was made, where 1 mg of FG contained 0.89 mg of dry gelatin. The mixed gelatin film with EDB (FGB) was prepared in a 1:1 ratio. EDB, FG, and FGB were tested for their antioxidant activity using DPPH. The microstructure of FG and FGB were observed using Scanning Electron Microscope. Wound healing treatment consisted of control without treatment (KN), control of povidone iodine (KP), drops of binahong leaf extract (TB), FG dressing, and FGB dressing, for 14 days with 3x observations (H2; H5; H14). Healthy skin on the first day of injury (H0) and skin on H-14 were made histological preparations using Masson's Trichrome staining then analyzed for histological semiquantitative values. The effect on wound diameter (mm) at the end of the treatment was treated with one-way ANOVA. Antioxidant activity (IC50) on EDB; FG; FGB respectively were 376.89 g/ml; not detected; 2434.47 ppm. The SEM microstructure of FG shows a smooth surface while FGB shows an uneven surface (there are bubbles and hollows). FG and FGB treatment had a significant effect on accelerating the wound healing process compared to without treatment. The application treatment of KIP gelatin film and binahong extract on wounds can increase the wound healing rate, especially in the inflammatory and maturation phases. As well as showing its effect on histological characteristics during the maturation phase (whereas at the dermis area observed the horizontally oriented and fascicle patterned of collagens, the presence of hair follicles, and thicker of epithelial tissue at the epidermis).
