Abstract

Tuberkulosis (TB) masih menjadi penyebab utama dari morbiditas dan mortalitas di negara berkembang. Diperkirakan sepertiga populasi di dunia telah terinfeksi Mycobacterium tuberculosis. Infeksi oleh M. tuberculosis diperparah dengan terjadinya resistansi yang menyebabkan pengobatan tuberkulosis semakin sulit dilakukan karena efektifitas obat akan semakin menurun. Gen rpoB pada subunit β RNA Polimerase dapat digunakan sebagai deteksi resistansi khususnya pada rifampicin. Kajian literatur ini menyajikan gambaran mengenai identifikasi dari mutasi gen rpoB terhadap obat TB serta kemungkinan efek mutasi yang akan terjadi pada RNAP (RNA Polimerase). Data yang berasal dari penelitian ilmiah disusun dan dianalisis secara deskriptif. Hasil kajian literatur menunjukkan bahwa substitusi nukleotida pada kodon 531, 526, dan 516 menjadi kodon terdampak mutasi paling tinggi, dengan juga terdapatnya kasus perbedaan hasil resistansi antara uji fenotipik dan genotipik (discordant, disputed, mutasi diam). Mutasi rpoB dapat menyebabkan berbagai perubahan interaksi seperti hilangnya ikatan h idrogen, interaksi hidrofobik yang hilang, hambatan sterik (steric hindrance) ataupun tolakan elektrostatik sehingga menyebabkan berkurangnya pelekatan RIF terhadap RNAP.

TB is still the main cause of morbidity and mortality in developing countries. It is estimated that one-third of the world's population has been infected by MTB. Infections caused by MTB are exacerbated by the occurrence of resistance, which makes TB treatment increasingly difficult because the effectiveness of the drug will decrease. The rpoB gene in the subunit of RNAP can be used as a resistance detection, especially for rifampicin. This literature review provides an overview of the identification of mutations that occur in the rpoB gene and the possible effects that will occur on RNAP (RNA Polimerase). Data from scientific research reports are compiled and analyzed descriptively. The results of the literature review showed that nucleotide substitution at codons 531, 526, and 516 are the most affected codon for mutations, with some cases of differences in resistance results between phenotypic and genotypic tests (discordant, disputed, and silent mutations). Mutation in rpoB can cause various interaction changes such as loss of hydrogen bonds, missing hydrophobic interactions, steric hindrance, or electrostatic repulsion, which can result in reduced RIF attachment to RNAP.
