Abstract

Dalam tugas akhir ini dilakukan analisis risiko kecelakaan kerja progressive pigging pipa bawah laut 12” MOL LCOM-NGLB di PT Pertamina Hulu Energi ONWJ yang bertujuan untuk mengetahui apa saja risiko kegagalan kecelakaan kerja yang terdapat pada proses progressive pigging serta frekuensi dan keparahan kegagalannya dengan menggunakan metode HAZOP. Tujuan dilakukannya tugas akhir ini adalah untuk mengetahui pengendalian terhadap dampak risiko yang dapat terjadi serta membahas konsekuensi kegagalannya. Penelitian ini dimulai dengan melakukan identifikasi hazard lalu dilanjutkan dengan penilaian risiko, kemudian menentukan langkah-langkah pengendalian potensi bahaya dominan yang ada. Dalam penelitian ini dilakukan penyebaran kuisioner kepada pihak terkait untuk mengetahui besaran frekuensi (frequency) dan keparahan (severity) potential hazard pada kecelakaan kerja progressive pigging. Dari hasil penelitian ini didapatkan bahwa potensi bahaya kecelakaan kerja saat proses progressive pigging yang termasuk kategori high risk adalah tekanan gas pada pipa yang terlalu tinggi pada aktivitas launching pigging, pelepasan gas mudah terbakar pada aktivitas launching pig, pelepasan gas beracun pada aktivitas launching pig, tekanan gas pada pipa yang terlalu tinggi pada aktivitas receiving pig, pelepasan gas mudah terbakar pada aktivitas receiving pig, dan pelepasan gas beracun pada aktivitas receiving pig.

In this final project, an analysis of the risk of progressive pigging accidents at work on the 12" MOL LCOM-NGLB underwater pipeline at PT Pertamina Hulu Energi ONWJ is carried out which aims to find out what the risks of work accident failure are in the progressive pigging process and the frequency and severity of failures by using the HAZOP. The purpose of doing this final project is to find out the controls for the impact of risks that can occur and to discuss the consequences of failure. This research begins with hazard identification and then continues with a risk assessment, then determines the steps to control the existing dominant potential hazards. In this study, questionnaires were distributed to related parties to determine the frequency and severity of potential hazards in progressive pigging work accidents. From the results of this study it was found that the potential hazard of work accidents during the progressive pigging process which is included in the high-risk category is the gas pressure in the pipe that is too high during launching pig activities, the release of flammable gas during launching pig activities, the release of toxic gas during launching pig activities, pressure gas in the pipeline that is too high in receiving pig activities, the release of flammable gas in receiving pig activities, and the release of toxic gases in receiving pig activities.
