Abstract
Dalam pemanfaatan laut melalui kegiatan-kegiatan seperti kepelabuhanan dan perikanan, diperlukan kapal yang dapat menjadi sarana berjalannya kegiatan dan salah satunya adalah kapal tunda. Menurut Undang-undang No. 17 Tahun 2008, kapal-kapal yang beroperasi wajib dikenakan perawatan berkala untuk menjaga keselamatan dan fungsionalitas kapal dalam beroperasi atau biasa disebut proses Docking. Dalam proses docking sendiri, tidak luput dari kegagalan operasi. Tujuan dari penulisan tugas akhir ini adalah untuk mengidentifikasi risiko dominan yang menyebabkan kegagalan pada proses operasi docking kapal, mencari penyebab dari risiko dominan, serta dapat mengetahui pengendalian risiko pengendalian risiko dominan yang tepat pada proses docking kapal. Penelitian ini dimulai dengan identifikasi bahaya menggunakan metode Fuzzy FMEA untuk mengetahui risiko mana yang dominan pada proses docking kapal. Kemudian dilakukan penyebaran kuisioner untuk mendapatkan nilai severity, occurrence, dan detection dari risiko kegagalan yang ada. Dari hasil penyebaran kuisioner dilakukan perhitungan Fuzzy Risk Priority Number pada setiap risiko untuk mengetahui risiko dominan dari jumalh FRPN tertinggi. Sehingga diketahui bahwa risiko kegagalan yang paling dominan adalah mata operator terkena gram. Hasil dari perhitungan FRPN kemudian dianalisis menggunakan metode FTA untuk mencari penyebab dari risiko kegagalan tersebut. Selanjutnya setelah mengetahui risiko dominan serta penyebabnya, dilakukan pemberian rekomendasi untuk mencegah terjadinya risiko kegagalan tersebut.

In utilizing the sea through activities such as ports and fisheries, a ship is needed that can be a means of carrying out activities and one of them is a tugboat. According to Law no. 17 of 2008, ships in operation must be subject to periodic maintenance to maintain the safety and functionality of the ship in operation or commonly known as the Docking process. In the process of docking itself, it does not escape from operating failures. The purpose of writing this final project is to identify the dominant risks that cause failure in the ship docking operation process, find the causes of the dominants, and be able to find out the right dominant risk control risk control in the ship docking process. This research begins with hazard assistance using the Fuzzy FMEA method to determine which risks are dominant in the ship docking process. Then a questionnaire was distributed to get the value of severity, occurrence, and detection of the existing risk of failure. From the results of distributing the questionnaires, a Fuzzy Risk Priority Number was calculated for each risk to determine the dominant risk of the highest FRPN number. So it is known that the most dominant risk offailure is the operator's eyes that are exposed to gram. The results of the FRPN calculations are then analyzed using the FTA method to find the causes of the risk of failure. Furthermore, after knowing the dominant risks and their resolution, recommendations are made to prevent the risk of failure occurring.
