Abstract

Keselamatan dan Kesehatan Kerja (K3) merupakan aspek penting dalam melakukan suatu pekerjaan. Dalam praktiknya, risiko potensi bahaya harus diidentifikasi guna mengurangi probabilitas terjadinya kecelakaan kerja. PT. XYZ merupakan industri pengolahan batu kapur yang belum terdapat risk assessment dan risk management. Penelitian ini mengidentifikasi potensi bahaya pada proses produksi di PT. XYZ dengan menggunakan metode Hazard and Operability (HAZOP). Selain itu juga dilakukan evaluasi keandalan pekerja dengan menggunakan metode Human Error Assessement and Reduction Technique (HEART) dan analisa beban kerja. Hasil analisa HAZOP didapatkan 1 risiko dengan kategori extreme, 5 risiko dengan kategori high, 19 risiko dengan kategori moderate dan 9 risiko dengan kategori low. Risiko yang termasuk kategori extreme adalah overload jaw crusher. Dari evaluasi keandalan manusia didapatkan hasil berupa 5 subtask ; 1 subtask pada bagian kerja bongkar muat, 1 subtask pada bagian kerja loader dan 3 subtask pada bagian kerja packing yang memiliki nilai Human Error Probability (HEP) diatas 100%. Hasil analisa beban kerja didapatkan nilai Workload Analysis (WLA) pada bagian kerja bongkar muat sebesar 33.52%, bagian kerja loader sebesar 26.37%, dan bagian kerja packing sebesar 21.27%. Antara nilai HEP dan WLA tidak memiliki keterikatan karena nilai WLA tidak ada yang melebihi 100%. Human error yang terjadi merupakan faktor kelalaian dan kegagalan dari manusia / para pekerja dan operator sendiri bukan dari beban kerja yang berlebihan.

Occupational Safety and Health (OSH) is an important aspect of doing a job. In practice, potential hazards must be identified in order to reduce the probability of work accidents occurring. PT. XYZ is a limestone processing industry that has not had risk assessment and risk management. This study identified potential hazards in the production process at PT. XYZ using the Hazard and Operability (HAZOP) method. In addition, employee reliability evaluation was also carried out using the Human Error Assessment and Reduction Technique (HEART) method and workload analysis. The results of the HAZOP analysis obtained 1 risk in the extreme category, 5 risks in the high category, 19 risks in the moderate category and 9 risks in the low category. The risk that belongs to the extreme category is the jaw crusher overload. From the evaluation of human reliability, the results obtained are 5 subtasks; 1 subtask in the loading and unloading work section, 1 subtasks in the loader work section and 3 subtasks in the packing work section that have a Human Error Probability (HEP) value above 100%. The results of the workload analysis showed that the Workload Analysis (WLA) value for the loading and unloading work was 33.52%, the loader work was 26.37%, and the packing work was 21.27%. There is no relation between the HEP and WLA values because the WLA value does not exceed 100%. Human error that occurs is a factor of negligence and failure of humans / workers and operators themselves, not from excessive workload.
