Abstract

Pada suatu proyek tentu nya terdapat berbagai macam kegiatan yang dapat menimbulkan ancaman risiko kecelakaan kerja. Pada penelitian tugas akhir ini bertujuan untuk mengidentifikasi risiko dominan apa saja yang dapat menyebabkan kecelakaan kerja pada proses loadout relokasi container crane (CC-05). Untuk dapat menyelesaikan permasalahan tersebut maka menggunakan metode Failure Mode and Effect Analysis (FMEA) untuk mmengidentifikasi bahaya dan risiko yang dominan pada proses loadout. Untuk mengetahui risiko dominan yang dihasilkan dilakukan perhitungan RPN. Melalui perhitungan RPN didapatkan 3 risiko dominan tertinggi yaitu tangan pekerja terjepit mesin winch dengan RPN 216, pekerja terkena luka bakar akibat tersengat listrik dengan RPN 200, tangan pekerja terkena luka bakar atau mata terpapar sinar las dan mengalami kerusakan dengan RPN 192. Hasil dari perhitunan RPN kemudian dianalisis menggunakan metode Fault Tree Analysis (FTA) untuk mencari penyebab dari risiko kecelekaan kerja tersebut, dari hasil pengkonstruksian FTA teridentifikasi 18 basic event. Dari analisis 2 metode tersebut dihasilkan 3 risiko dominan yaitu Tangan terjepit mesin winch, Pekerja terkena luka bakar akibat tersengat listrik dan Mata terpapar sinar las. Tiga risiko dominan tersebut selanjutnya dilakukan pengkategorian pada matriks risiko. Pada matriks risiko menunjukan 3 risiko tersebut masih tergolong not acceptable dan harus dilakukan rekomendasi untuk mencegah terjadinya kecelakaan kerja tersebut

In a project, of course, there are various kinds of activities that can pose a risk of work accidents. In this final project research aims to identify what are the dominant risks that can cause work accidents in the container crane relocation loadout process (CC-05). To be able to solve these problems, the Failure Mode and Effect Analysis (FMEA) method is used to identify the dominant hazards and risks in the loadout process. To find out the dominant risk generated, the RPN calculation is carried out. Through the RPN calculation, the 3 highest dominant risks are obtained, namely the worker's hand is caught by a winch machine with RPN 216, the worker gets burns due to electric shock with RPN 200, the worker's hand gets burned or the eye is exposed to the welding beam and suffers damage with RPN 192. The results of the RPN calculation then analyzed using the Fault Tree Analysis (FTA) method to find the causes of the work accident risk, from the results of the construction of the FTA identified 18 basic events. From the analysis of the 2 methods, 3 dominant risks were generated, namely the hand being caught by the winch machine, workers getting burns due to electric shock and eyes exposed to welding rays. The three dominant risks are then categorized in the risk matrix. The risk matrix shows that these 3 risks are still classified as not acceptable and recommendations must be made to prevent these work accidents from happening.
