Abstract

Pipeline merupakan satu teknologi yang memiliki tujuan untuk mengalirkan gas maupun fluida seperti minyak, gas, dan air dalam jumlah yang banyak serta dari jarak yang jauh. Penggunaan pipeline secara terus menerus tidak lepas dari berbagai faktor yang dapat menyebabkan kegagalan operasi. Pada penelitian ini dilakukan analisis risiko operasi pipeline milik PT XYZ dengan menggunakan metode Fuzzy Failure Mode and Effect Analysis (FFMEA). Hasil analisis FFMEA menunjukkan Fuzzy Risk Priority Number (FRPN) tertinggi dengan nilai 7,47 pada mode kegagalan impact of anchor dan failure effect berupa kebocoran pada pipa. Untuk mengetahui akar - akar penyebab kegagalan operasi pada pipeline analisis dilanjutkan menggunakan metode Fault Tree Analysis (FTA). Hasil analisis menggunakan Metode FTA diketahui penyebab dari kebocoran pipa dengan nilai probabilitas tertinggi yakni aktivitas jangkar dengan nilai sebesar 0,013250. Kemudian pada penelitian ini juga memberikan rekomendasi untuk meminimalisir risiko yang dapat menyebabkan kegagalan operasi pada pipeline.

Pipeline is a technology that has aim of flowing gas and fluids such as oil, gas, and water in large quantities and form a long distance. Continuous use of pipelines cannot be separated from various factors that can cause operating failures. In this study, a risk analysis of pipeline operations owned by PT XYZ was carried out using the Fuzzy Failure Mode and Effect Analysis (FFMEA) method. The result of the FFMEA analysis show the highest Fuzzy Risk Priority Number (RPN) with value of 7,47 in the failure mode impact of anchor and failure effect in the form of leakage in the pipe. To find out the root causes of operating failures on the pipeline, the analysis is continued using Fault Tree Analysis (FTA) method. The result of the analysis using the FTA method show that the cause of pipe leakage with the highest probability value is anchor activity with a value of 0,013250. Then this study also provides recommendations for minimizing risks that can cause operating failures on the pipeline.
