Abstract
PT Otsuka Indonesia merupakan perusahaan yang bergerak di bidang industri farmasi dengan memproduksi dan memasarkan cairan infus dan obat etikal, maupun produk nutrisi untuk keperluan medis khusus serta alat kesehatan. Dari 6 divisi produksi yang ada, salah satu divisi produk yang khusus untuk memproduksi produk infus yaitu Plastic Ampoule. Dalam proses produksi ditemukan beberapa defect yang dapat mempengaruhi kepuasan pelanggan serta berdampak pada produktivitas perusahaan. Penelitian ini berfokus pada Otsu WI 25 ml karena merupakan backbone produk dari plastic ampoule dimana Otsu WI menghasilkan jumlah produksi defect terbanyak sebesar 139.470 dari 164.492 defect plastic ampoule dan proporsi cacat tertinggi diantara produk lainnya yaitu sebesar 1.6% pada rentang Oktober 2022 – Februari 2023. Pengendalian kualitas dilakukan menggunakan metode Six sigma DMAIC untuk melakukan perbaikan yang bertujuan untuk mereduksi jumlah cacat yang terjadi. Pada tahap define dilakukan identifikasi proses produksi Otsu WI 25 ml serta mengidentifikasi permasalahan kualitas yang terjadi pada proses produksi. Hal ini dilakukan dengan tujuan untuk mengetahui proses yang perlu dilakukan improvement. Pada tahap measure dilakukan perhitungan six sigma untuk mengukur kinerja proses pada kondisi existing. Selanjutnya pada tahap analyze dan improvement akan dilakukan analisis berdasarkan perhitungan metode six sigma menggunakan Root Cause Analysis untuk mengetahui akar penyebab terjadinya defect dan pemilihan alternatif perbaikan berdasarkan nilai Risk Priority Number tertinggi pada Failure Mode Effect and Analysis. Dikethui root cause dari defect terkritis yaitu terdapat kotoran pada die ring dan jalur vacuum yang tersumbat. Terdapat dua alternatif perbaikan yang diusulkan yaitu pembersihan serta monitoring suhu pada die ring dan pembersihan jalur vacuum. Selain itu, juga didapatkan peningkatan nilai sigma pada kondisi existing dari 4,35 menjadi 4,63 pada kondisi improvement.

PT Otsuka Indonesia is a company engaged in the pharmaceutical industry by producing and marketing infusion or injection fluids and ethical drugs, as well as nutritional products for special medical purposes and medical device products. From the 6 existing production divisions, one of them are product divisions specifically for producing infusion products, namely the Plastic Ampoule divisions. In the production process, there are several defects that can affect customer satisfaction and have an impact on company productivity. So the purpose of this study is to reduce defects that occur in the production process of Plastic Ampoule Otsu WI 25ml with a six sigma approach. This research focuses on Otsu WI 25 ml because it is the backbone product of plastic ampoules where Otsu WI produces the highest number of defect production of 139470 out of 164492 plastic ampoule defects and the highest proportion of defects among other products which is 1.6% in the range of October 2022 - February 2023. Quality control is carried out using the Six Sigma DMAIC method to make improvements aimed at reducing the number of defects that occur. In the define stage, the production process of Otsu WI 25 ml is identified and quality problems that occur in the production process are identified. This is done with the aim of knowing the process that needs improvement. At the measure stage, six sigma calculations are carried out to measure process performance in existing conditions. Furthermore, at the analyze and improvement stage, an analysis will be carried out based on the calculation of the six sigma method using Root Cause Analysis to determine the root cause of the defect and the selection of alternative improvements based on the highest Risk Priority Number value in Failure Mode Effect and Analysis. The root cause of the most critical defect was found to be dirt on the die ring and a clogged vacuum line. There are two alternative improvements proposed, namely cleaning and monitoring the temperature of the die ring and cleaning the vacuum line. In addition, there is also an increase in the sigma value in the existing condition from 4.35 to 4.63 in the improvement condition.
