Abstract

This research will be conducted to measure the reliability of ship captains as a mitigation for the occurrence of human errors in operating tugboats, using the technique for human errorrate prediction (THERP) method at PT. Nisrina Rizki Samudera. The results obtained from this research show that the highest human error probabilities (HEP) are associated with the operation of controlling tugboats in adverse weather conditions. This indicates that 7% of the total sub-operations performed by ship captains in controlling tugboats result in an 11% risk of human error. In this sub-operation, there are various tasks involving decision-making, including performing minor actions, major actions, and reporting emergency situations. Based on the HEP values, recommendations for improvement are provided to the company to reduce errors in the operation of controlling tugboats in adverse weather conditions. For example, one recommendation is to implement an early warning system design that is expected to help reduce the likelihood of human errors in the decision-making process of ship captains during adverse weather conditions.
