Abstract

Melalui proses pemanasan, boiler mengubah air menjadi uap.
Proses tersebut tentu mempunyai potensi bahaya. Salah satu faktor
penyebab terjadinya bahaya adalah faktor manusia dan kegagalan
instrumen. Analisis ini dilakukan pada faktor manusia terhadap
kegagalan instrumen. Dengan menggunakan data maintenance
instrumen dan observasi pekerja melalui kuesioner, maka didapatkan
Mean Time To Failure (MTTF), laju kegagalan, serta Probability
Failure on Demand (PFD) setiap instrumen. Metode regresi digunakan
untuk menentukan hubungan kedua variabel tersebut, yaitu faktor
manusia dan kegagalan instrumen yang diwakili oleh nilai PFD. Dari
hasil perhitungan, didapatkan bahwa kegagalan instrumen dipengaruhi
oleh faktor internal dan faktor eksternal manusia. Untuk faktor internal
diantaranya adalah kepercayaan diri, perhatian, kemampuan,
penyelesaian masalah, bekerja di bawah tekanan, kelalaian, dan stres
pekerjaan menumpuk. Dan faktor eksternal yaitu informasi maupun
standar yang diberikan oleh perusahaan (procedures), pendidikan dan
pelatihan, komunikasi, waktu, serta evaluasi terhadap kegagalan
instrumen di boiler PT. Pertamina (Persero) Unit Pengolahan IV
Cilacap. Besar pengaruh (penyimpangan) faktor manusia terhadap
kegagalan instrumen dapat ditunjukan melalui nilai U. Faktor yang
paling berpengaruh dari manusia terhadap kegagalan instrumen pada
boiler di PT. Pertamina (Persero) Unit Pengolahan IV Cilacap adalah
task load stress yang mempunyai nilai U paling kecil di antara faktor
yang lainnya yaitu sebesar 0,0866. Hal ini selaras dengan faktor
internal yang mempunyai nilai U yang lebih kecil dari pada faktor
eksternal yaitu sebesar 0,0953.

Through the heating process, boiler converts water into steam. The
process certainly has the potential dangers. One of factors contributing
to the danger is the human factor and instrument failure. The analysis
was performed on the human factor to the instrument failure. By using
the maintenance data of instrument and observation workers through
the questionnaire, the Mean Time To Failure (MTTF), the failure ratee,
and Probability Failure on Demand (PFD) of each instrument are
obtained. Regression method used to determine the relationship
between the two variables, namely the human factor and the instrument
failure which represented by the PFD value. From the calculation, it
was found that the instrument failure is influenced by internal factors
and external factors of human. For internal factors include selfconfidence,
attention, skill, problem solving, working under pressure,
omission, and the task load stress. And external factors include the
information that is provided by the company's standards (procedures),
education and training, communication, time, as well as an evaluation
of instrument failures in boiler PT. Pertamina (Persero) Refinery Unit
IV Cilacap. Large effect (deviation) of the human factor to the
instrument failure can be indicated by the value of U. The most
influential factor of human to the instrument failure in the boiler at PT.
Pertamina (Persero) Refinery Unit IV Cilacap is the task load of stress
that has the smallest value of U among other factors is equal to 0.0866.
This is consistent with the internal factors that have a U value that is
smaller than the external factor is equal to 0.0953.
