Abstract

International Maritime Organization (IMO) telah mempelajari bahwa terdapat
banyak kejadian di mana pengirim barang tidak menyatakan berat peti kemas sesuai
dengan berat aslinya demi menghindari biaya pengiriman yang mahal atau terdapat
kegagalan dalam proses penimbangan. IMO kemudian membuat amandemen
mengenai Safety of Life at Sea (SOLAS), mengenai informasi peti kemas yang
tercantum secara khusus di SOLAS Bab VI: "Carriage of Cargo". Amandemen ini
dikenal sebagai amandemen Verified Gross Mass (VGM). VGM telah efektif sejak 1
Juli 2016 dan mengakibatkan risiko kecelakaan peti kemas berkurang. Meskipun
VGM telah meningkatkan keamanan pada kapal kontainer, banyak pertanyaan yang
ditimbulkan mengenai akurasi dan keandalan VGM sendiri dalam melaporkan berat
aktual peti kemas. Masalah ini mengakibatkan banyak kerugian sebagaimana berat
peti kemas tidak dilaporkan secara akurat. Munculnya VGM tidak akan berarti
apabila tetap mengakibatkan kecelakaan atau kegagalan. Sebuah model mengenai
manajemen risiko dapat mengidentifikasi faktor risiko dalam proses ini. House of
Risk Model (HOR) dapat mengidentifikasi agen risiko, peristiwa risiko, dengan
melibatkan Failure Model Event Analysis (FMEA) dalam pengambilan sampel data
berdasarkan karakteristik FMEA. Tugas akhir ini akan diterapkan berdasarkan
peristiwa risiko dan agen risiko yang terjadi pada peti kemas di Terminal Teluk
Lamong. Pendekatan mitigasi yang akan dilakukan adalah dengan menggunakan
aturan Pareto. Hasil dari tugas akhir ini dapat menjadi pengetahuan lebih lanjut
mengenai cara yang lebih efektif dalam mengurangi kegagalan pada pengukuran
berat peti kemas.

International Maritime Organization (IMO) has learned that there were many
situations where cargo owners or shippers did not declare the actual weight of their
goods, due to avoiding higher cost in shipping. IMO then made an amendment for
the convention of Safety of Life at Sea (SOLAS) about the cargo information which
is listed specifically in SOLAS Chapter VI: “Carriage of Cargoes”. The amendment
is well known as the Verified Gross Mass (VGM) amendment. Since then, VGM has
been effective since 1st July 2016 resulting a good differences in reducing the risk of
accidents in container handling. Although VGM has improve the safety major of
container shipping, problems were keep arising regarding on its accuracy and
reliability in measuring the actual measurement. This problem has resulted a lot of
loss as a container was not declared accurately. The appearance of VGM would be
useless as it does not declaring the accurate weight and will suffer the probability of
accidents and further failures. A model for proactive risk management is choose as
the model would be able to identify the risk factor in this process. By using the House
of Risk Model (HOR) to identify the risk agents, risk event, and involving the Failure
Model Event Analysis (FMEA) for sampling the data based on FMEA characteristics.
The result of this thesis will determine based on the level of the risk and correlation
between risk event and risk agent that happened for containers in Teluk Lamong
Terminal. A mitigation approach is conducted using a Pareto Rules. The thesis will
be resulted a further improvement and knowledge for reducing failures in container
weight measurement.
