Abstract

Tangki penyimpanan kondensat memiliki fungsi yang sangat penting dalam mengatur arus minyak di PT. Trans Pacific Petrochemical Indotama (PT TPPI). Jumlah dan volume tangki yang terbatas untuk menyimpan kondensat dengan berbagai jenis yang menyesuaikan kondisi Kilang yang beroperasi terus menerus, menuntut kesiapan tangki yang optimal setiap saat. Disamping merusak fasilitas kilang, kebakaran tangki penyimpanan baban baku di kilang minyak, dapat menyebabkan kematian atau cedera bagi pekerja dan pencemaran minyak ke lingkungan yang dapat menimbulkan kerugian besar bagi perusahaan. Penelitian ini bertujuan untuk melakukan analisis bahaya yang mendalam dan memadai di fasilitas penyimpanan kondensat di PT TPPI. Pada tahap awal, potensi failure yang tidak diinginkan dalam operasional tangki kondensat di PT TPPI diidentifikasi melalui focus group discussion yang di dominasi staff pada bagian tank terminal PT TPPI dan selanjutnya menggunakan metode Risk Failure Mode Effect and Analysis (RFMEA) hingga didapat bahwa risiko kebakaran tangki kondensat merupakan mode kegagalan yang paling kritikal dalam operasional tangki. Tahap selanjutnya adalah mencari faktor-faktor risiko kritikal untuk kebakaran tangki kondensat dengan menggunakan Fault Tree Analysis (FTA). Dari diagram FTA, diperoleh 35 basic event yang merupakan faktor risiko serta 10 minimum path sets (MPS) yang harus diputus untuk untuk mencegah terjadinya top event (risiko kebakaran tangki). Dengan pendekatan fuzzy triangular hasil analisa faktor risiko kualitatif dari FTA yang tidak memiliki propabilitas yang pasti, diubah menjadi kuantitatif sehingga diketahui level faktor risikonya dengan penilaian tiga orang ahli yang dinormalisasi dengan cara 3σ. Kebocoran seal, vapor yang terpapar ke udara kering, korosi pada pipa atau nozzle, grounding system dan hantaman lightning merupakan faktor risiko yang paling tinggi dari risiko kebakaran tangki kondensat di PT TPPI. Pada akhirnya dapat disampaikan beberapa tindakan pencegahan dan mitigasi untuk mengurangi dampak terjadinya kebakaran tanki penyimpanan kondensat di PT TPPI. ================================================================== The condensate storage tank has a very important function in regulating the flow of oil in PT. Trans Pacific Petrochemical Indotama (PT TPPI). The limited number and volume of tanks to store condensate with various types that should be able to follow the operation condition of the refineries that operate continuously requires the optimal tank readiness at all the times. In addition to damaging the refinery facilities, fire of oil storage tank at an oil refinery can cause death or injury to workers and oil pollutant to the environment that could gave big losses to the company. This study aims to conduct an in-depth and adequate hazard analysis at the condensate storage facility at PT TPPI. In the initial stage, the potential for failure in condensate tank operations in PT TPPI was identified through focus group discussions that dominated by staff at the PT TPPI’s tank terminal and subsequently using the Risk Failure Mode Effect and Analysis (RFMEA) method till it was found that fire risk of condensate tank is the most critical failure mode in the tank operations. The next step is to look for critical risk factors for fire risk of condensate tank by using Fault Tree Analysis (FTA). From the FTA diagram, 35 basic events are obtained which are risk factors and 10 minimum path sets (MPS) that must be disconnected to prevent top events exist (tank fire risk). With the fuzzy triangular approach, the qualitative risk factor analysis results from the FTA that do not have a definite propability, are converted to quantitative so that the risk factor level that is known by the assessment of three experts normalized by 3σ method. Seal leaks, vapors exposed to dry air, corrosion in pipes or nozzles, grounding systems and lightning strike are the highest risk factors of fire risk in condensate tanks at PT TPPI. Finally, several prevention and mitigation measures can be delivered to reduce the impact of a condensate storage tank fire at PT TPPI.
