Abstract

Kanker serviks adalah penyakit ginekologi yang memiliki tingkat keganasan dan disebabkan oleh Human Papilloma Virus (HPV). Penelitian ini bertujuan untuk mengetahui karakteristik dari pasien kanker serviks di RSUD Dr. Soetomo dan untuk mengetahui model terbaik antara bayesian cox stratifikasi dengan prior noninformatif dan prior informatif serta model dari cox stratifikasi tanpa pendekatan bayesian. Penelitian ini menggunakan data pasien kanker serviks rawat inap di RSUD Dr. Soetomo pada tahun 2014. Analisis dilakukan pada faktor-faktor yang diduga mempengaruhi peluang hidup pasien kanker serviks, diantaranya faktor usia, stadium, jenis pengobatan, penyakit penyerta, komplikasi, dan status anemia. Berdasarkan dari satu tahun penelitian dengan jumlah pasien sebanyak 746 pasien terdapat 36 pasien meninggal dan 710 pasien yang bertahan. Kurva Kaplan Meier menunjukkan bahwa probabilitas ketahanan hidup penderita kanker serviks pada jangka waktu satu tahun masih tinggi, yaitu diatas 0,75. Berdasarkan uji Log Rank diketahui bahwa pada variabel usia tidak ada perbedaan peluang bertahan hidup antara pasien usia dewasa (24-45 tahun) dengan usia lansia (usia 46 tahun keatas), sedangkan pada variabel stadium terdapat perbedaan probabilitas bertahan hidup selama satu tahun antara pasien stadium 0 sampai stadium 4, pada variabel jenis pengobatan terdapat perbedaan probabilitas bertahan hidup pada pasien yang menggunakan pengobatan kemoterapi, transfuse PRC, dan kombinasi keduanya. Probabilitas bertahan hidup selama satu tahun pada pasien dengan kanker serviks sebagai pernyakit utama berbeda dengan pasien kanker serviks sebagai penyakit penyerta. Pasien dengan komplikasi memiliki peluang hidup lebih rendah daripada pasien tanpa komplikasi dalam rentang waktu satu tahun pengamatan. Pasien dengan anemia memiliki peluang hidup lebih rendah daripada pasien tanpa anemia dalam waktu pengamatan satu tahun. Analisis dengan menggunakan cox stratifikasi terhadap variabel stadium yang menunjukkan bahwa hanya variabel komplikasi yang berpengaruh signifikan terhadap model. Selanjutnya dilakukan pendekatan menggunakan pendekatan bayesian dengan prior noninformatif dan informatif. Perbandingan antara cox stratifikasi dengan bayesian cox stratifikasi diketahui bahwa model bayesian lebih baik. Pada pendekatan bayesian dengan dua prior yang berbeda didapatkan hasil yang sama. Hal ini disebabkan karena periode data yang singkat.

Cervical cancer is a gynecology disease that causes by Human Papillomavirus (HPV) has different malignancy. The purposes of this research are to find out the characteristic of cervical cancer patients in Dr. Soetomo General Hospital and also find out the best model between Bayesian Cox stratification with non-informative priors and informative prior also Cox stratification model without Bayesian approach. This research is using inpatient treatment data of cervical cancer patients on Dr. Soetomo General Hospital in 2014. The analysis was looking at some factors that could increase cervical cancer patients’ chance of surviving, including age, stage, type of treatment, another patients’ coexist disease, complication, and anemia status. According to a year research with 746 patients, there are 36 patients could not be survived and the rest of 710 patients could be survived. Kaplan-Meier curve indicates that the survival probability of cervical cancer patient in a year still high which is more than 0, 75. According to Log Rank test, there are known that age variable between adult (24-45 years old) and elder (more than 46 years old) have no difference in chance of survival, although, on stage variable the chance of survival between early-stage patients and terminal stage patients is different, the treatment variable has given different chance of survival for patients that using chemotherapy, transfuse PRC, and both of them. The chances of survival between cervical cancer as a main disease and as a coexisting disease are different. According to a year research, patients with complication have a smaller chance of surviving than the patients without it. Patients who having an anemia also have smaller chances of surviving than patients who do not having it. The Cox stratification analysis that used on stage variable showed that complication is the only variable which has a significant influence on the model. After that, the Bayesian prior non-informative and informative approach is used. A comparison between Bayesian Cox stratification and Cox stratification is known that Bayesian model better. On Bayesian approaches that use two different priors, there is having the same results. A short period of data is the cause.
