Abstract

Limbah medis adalah salah satu masalah lingkungan yang penting di dunia, terutama di Surabaya. Hal tersebut dikarenakan bahaya lingkungan dan risiko kesehatan. Mengolah dan mengangkut limbah medis menjadi penting karena ada peraturan yang mengatur tentang pelaksanaannya.Dinas Kesehatan Kota Surabaya memutuskan untuk menggunakan jasa pihak ketiga untuk mengolah dan mengangkut limbah medis dari 63 puskesmas di Kota Surabaya. Pada kondisi sekarang, pengangkutan limbah dilakukan rata-rata sebulan sekali. Namun, terdapat peraturan yang mengharuskan limbah diangkut tidak lebih dari 14 hari semenjak pertama kali limbah tersebut dibuang di tempat pembuangan sementara (TPS). Puskesmas sebenarnya dapat meminta pihak pengolah limbah untuk mengangkut limbahnya lebih cepat, dari sebulan sekali menjadi paling tidak setiap 14 hari sekali. Akan tetapi pihak pengolah mempunyai batas minimal 25 kilogram limbah terakumulasi untuk dapat melakukan pengangkutan, hal ini disebabkan karena adanya prinsip break-even dari pengolah antara biaya jasa yang dibayarkan per kilogram limbah dan biaya transportasi untuk mengangkut limbah medis di lokasi puskesmas.
Riset ini mengevaluasi batas 25 kilogram dari pihak pengolah dan menganalisis implementasi pengangkutan limbah setiap 14 hari. Periodic Vehicle Routing Problem (PVRP) adalah tools yang dipilih untuk merencanakan pengangkutan limbah medis. Dinas Kesehatan Kota Surabaya akan dapat merencanakan kontrak dengan pihak pengolah limbah dengan satu tahun masa kontrak. Skenario dikembangan untuk mengkonsiderasi kemungkinan atau alternatif lain mengenai praktik pengangkutan limbah medis.
Dalam riset ini ditemukan bahwa pengangkutan limbah medis setiap 14 hari akan menguntungkan pihak Dinas Kesehatan Kota Surabaya dan pihak pengolah limbah medis karena dengan menerapkan hal tersebut peraturan tentang maksimum masa penyimpanan selama 14 hari akan dipenuhi dan total jarak tempuh kendaraan pihak pengolah akan lebih pendek dibandingkan dengan kondisi sekarang.

Medical waste is one of the most important environmental problems in the world, particularly in Surabaya. It is because of the potential environmental hazards and public health risks. Treating and transporting the infectious medical waste are important since it requires regulations to follow. Surabaya Council for Health decided to use third party waste processing vendors to treat and transport infectious waste from 63 health facilities in Surabaya. The current waste pick-up is done once a month for majority of health facilities. However, there are 14 days waste storing regulations to follow. Health facilities actually can request vendor to pick-up their waste more frequently but the vendor has 25 kilograms threshold of picking-up the waste per health facility. In other word the vendor won’t pick the waste when it is less than 25 kilograms. it is due to break-even principle from vendor from point of view regarding the price they got of service they provide per kilogram and the cost of transportation when picking up the waste.
This research is trying to evaluate the 25 kilograms threshold from the vendor and analyzing the 14 days storing duration implementation. Periodic Vehicle Routing Problem (PVRP) is a tool chosen in planning the medical waste transportation. Surabaya City Council will be able to plan the contract with the waste processing vendor in a yearly planning horizon. Scenario was developed and tested to consider other possibilities regarding the system.
From this research, it is concluded that 14 days storing duration of medical waste might benefit both Surabaya City Council for Health since it is complying to the storing regulation and also benefit waste processing vendor because it is resulted in less total distance traveled compared with when using no maximum storing duration.
