Abstract

Penilaian risiko dan cara pengendaliannya merupakan hal yang sangat penting dalam pengerjaan proyek instalasi bangunan lepas pantai, karena dalam pelaksanaannya sangat rentan terhadap risiko kecelakaan kerja. Penelitian ini menggunakan studi kasus pengerjaan instalasi jacket structure pada proyek reaktivasi salah satu platform milik perusahaan minyak dan gas di Indonesia, yang berlokasi di Selat Madura. Pada penelitian ini dilakukan analisis risiko untuk mengetahui risiko kecelakaan kerja yang dapat terjadi dalam proses instalasi jacket structure dengan menggunakan metode Failure Mode and Effect Analysis (FMEA), dan langkah mitigasinya dengan menggunakan metode Fault Tree Analysis (FTA). Dari hasil penelitian ini didapatkan bahwa risiko kecelakaan yang dominan terjadi adalah floating transportation barge menghantam crane vessel, kepala pekerja terbentur besi pile, dan pekerja terjatuh dari basket crew.

Risk assessment and how to control it is a very important things on offshore
platform installation project operation due to it vulnerability to risk of work accident
on the execution. On this thesis, a jacket structure installation process on
reactivation project of platform who owned by one of Indonesian Oil and Gas
Company that located on Madura starit is be used. On this thesis, risk assessment
will be carried out using two methods, to find out risk of accident that can occur
during this jacket structure installation process the author is using Failure Mode
and Effect Analysis (FTA) method, and to find out how to mitigate the risk the
author is using fault Tree Analysis (FTA) method. The results from this research is
be derived that the dominant risks that can occure during the procces mentioned are
the floating transportation barge is raming the crane vessel, worker’s head are hit
by pile, and worker fell of the basket crew.
