Abstract

Kegiatan pengisian bahan bakar LPG merupakan kegiatan yang berpotensi menimbulkan bahaya penyebab penyakit dan kecelakaan kerja. Potensi risiko bahaya tidak lepas dari beberapa faktor yang disebabkan oleh kelalaian atau ketidakpatuhan pekerja terhadap prosedur K3. Sehingga diperlukan manajemen risiko sebagai bentuk pengendalian terhadap potensi risiko bahaya. Penelitian ini menggunakan metode Hazard Identification, Risk Assessment, and Risk Control (HIRARC) dengan tujuan untuk mengidentifikasi risiko, faktor penyebab dan dampak dari risiko tersebut. Analisis lebih lanjut dilakukan dalam bentuk kombinasi framework lean manufacturing Fuzzy Bow-tie. Model Bow-tie dirancang untuk mempertimbangkan faktor-faktor risiko yang dapat menyebabkan risiko dengan pohon kesalahan, dan kemudian dampak risiko dijelaskan dalam pohon kejadian. Kemudian analisis fuzzy digunakan untuk menghitung probabilitas faktor dan dampak yang terjadi. Dan yang terakhir menggunakan matriks risiko untuk menentukan potensi risiko. Pada penelitian ini ada 5 risiko utama yang teridentifikasi, dengan total 13 faktor dan 12 dampak risiko. Risiko yang menjadi prioritas berada pada level High Risk yaitu, risiko kebakaran dan risiko paparan gas. Risiko pada level High Risk (prioritas) diberikan rekomendasi mitigasi berupa tindakan preventif dan protektif. Kontribusi penelitian ini dalam bidang akademik yaitu bagaimana penggunaan metode HIRARC dan analisis fuzzy bow-tie dalam memetakan dan menentukan risiko prioritas pada suatu perusahaan. Sedangkan bagi perusahaan penelitian ini dapat digunakan sebagai acuan untuk membantu perusahaan dalam pengendalian dan antisipasi potensi bahaya.

The activity of refueling LPG is an activity that has a potential risk of hazards causing occupational diseases and accidents. The potential risk of danger cannot be separated from several factors caused by negligence or non-compliance of workers with K3 procedures. So a risk management is needed as a form of control over potential hazard risks. This research uses the Hazard Identification, Risk Assessment, and Risk Control (HIRARC) method with the aim of identifying risks, the factors that cause and the impact of these risks. Further analysis is carried out in the form of a combination of Fuzzy Bow-tie lean manufacturing framework. The Bow-tie model is designed to consider the risk factors that can cause risk with the fault tree, and then the impact of the risk is described in the event tree. Then fuzzy analysis is used to calculate the probability of the factors and impacts that occur. And the last one uses a risk matrix to determine potential risks. In this study, there are 5 main risks identified with a total of 13 factors and 12 risk impacts. The priority risks are at the High Risk level, namely, fire risk and gas exposure risk. Risk at the High Risk (priority) level recommends mitigation recommendations in the form of preventive and protective actions. The contribution of this research in the academic field, namely how to use the HIRARC method and the fuzzy bow-tie analysis in mapping and determining priority risk in a company. Meanwhile, for companies this research can be used as a reference to assist companies in controlling and anticipating potential hazards.
