Abstract
Persaingan bisnis saat ini semakin ketat dan menyebabkan supply chain dalam perusahaan menjadi semakin kompleks. PT Tirta Investama Klaten merupakan salah satu perusahaan air minum dalam kemasan (AMDK) di Indonesia. Dalam aliran rantai pasoknya, terdapat risiko yang perlu dimitigasi agar aktivitas rantai pasok berjalan dengan baik dan dapat bersaing dengan kompetitor. Penelitian ini dilakukan untuk mengidentifikasi risiko dan penyebabnya, serta memberikan tindakan pencegahan risiko. Untuk memitigasi risiko, maka digunakan manajemen risiko yang disesuaikan dengan standar manajemen risiko ISO 31000:2018. Model Supply Chain Operations Reference (SCOR) digunakan untuk mengidentifikasi aktivitas rantai pasok yang terdiri dari proses source, make, dan deliver, dan didapatkan 27 aktivitas rantai pasok yang terdiri dari 7 aktivitas pada proses source, 17 aktivitas pada proses make, dan 3 aktivitas pada proses deliver. Dalam penelitian ini digunakan model House of Risk (HOR) untuk menganalisis risiko rantai pasok yang terdiri dari 2 fase. Pada model HOR fase 1, didapatkan kejadian risiko (risk event) sebanyak 90 kejadian risiko, yang terdiri dari 21 kejadian risiko pada proses source, 62 kejadian risiko pada proses make, dan 7 kejadian risiko pada proses deliver, dan didapatkan agen risiko (risk agent) sebanyak 28 agen risiko. Hasil dari diagram pareto 80:20 menunjukkan terdapat 14 agen risiko yang menjadi prioritas untuk diberikan tindakan pencegahan risiko. Pada model HOR fase 2, diberikan 24 tindakan pencegahan risiko untuk mengurangi kemungkinan terjadinya agen risiko.

Business competition getting tighter nowadays and causes the supply chain within the company become complex. PT Tirta Investama Klaten is one of the bottled drinking water companies in Indonesia. In their supply chain network, there are risks that need to be mitigated so that supply chain activities run well and can compete with competitors. This study was conducted to identify risks and their causes, and provide risk preventive actions. To mitigate risk, risk management is used and adjusted to the ISO 31000:2018 risk management standard. The Supply Chain Operations Reference (SCOR) model is used to identify supply chain activities consisting of source, make, and deliver processes, and 27 supply chain activities obtained consisting of 7 activities of source process, 17 activities of make process, and 3 activities of deliver process. In this study, the House of Risk (HOR) model is used to analyze supply chain risk which consists of 2 phases. In the HOR model phase 1, 90 risk events were identified, consisting of 21 risk events of source process, 62 risk events of make process, and 7 risk events of deliver process, and 28 risk agents were also obtained. The results of the 80:20 pareto diagram shows that there are 14 risk agents that are prioritized for risk preventive actions. In the HOR model phase 2, 24 risk preventive actions are given to reduce the possibility of the occurrence of risk agents.
