Abstract

Indonesia, sebagai salah satu negara dengan curah hujan yang cukup tinggi.
Penyakit ini ditularkan melalui nyamuk Aedes Aegypty, salah satu jenis nyamuk yang
berkembang di negara tropis. Jenis nyamuk ini dapat ditemukan di seluruh wilayah
Indonesia, termasuk provinsi Jawa Timur, khususnya Kabupaten Pamekasan (Madura)
dengan jumlah penderita DBD semakin meningkat sejak Januari 2009. Jumlah penderita
DBD meningkat tiap tahun, ribuan masyarakat terserang penyakit ini dan ratusan
diantaranya meninggal tiap tahunnya. Maka dari itu, perlu dilakukan penelitian untuk
mengetahui laju kesembuhan pasien dan faktor-faktor yang mempengaruhinya. Faktor
yang diduga berpengaruh terhadap laju kesembuhan pasien DHF meliputi usia, jenis
kelamin, kadar hematokrit, dan jumlah trombosit. Penelitian ini menggunakan metode
Bayesian Mixture Survival Spasial untuk menganalisis faktor-faktor yang mempengaruhi
laju kesembuhan pasien DBD yang berasal dari RS Pamekasan, Madura. Model
menyertakan efek random/frailty spasial CAR (Conditionally Autoregressive) untuk
mengatasi kebergantungan spasial yang tidak terjelaskan dalam model. Penelitian ini
menggunakan matriks pembobot Queen’s contiguity. Dalam menentukan ada/tidaknya
pengaruh antar kecamatan (lokasi) terhadap angka kejadian DBD maka digunakan
Statistik Uji Moran’s I yang menunjukkan nilai statistik Moran’s I sebesar 0.1993 dan
nilai Z-value sebesar 2.411 sehingga disimpulkan bahwa terdapat pengaruh ruang yang
signifikan pada kejadian DBD di tiap kecamatan di Pamekasan. Distribusi weibull 2-
parameter merupakan distribusi yang paling sesuai untuk memodelkan laju kesembuhan
pasien DBD. Pada kasus ini terdapat kasus bi-modal yaitu komponen mixture pertama
memiliki puncak pada t=4 dan komponen mixture kedua memiliki puncak pada t=8.
Fungsi survival pasien DBD menurun seiring lamanya waktu rawat inap, sedangkan
fungsi hazard semakin meningkat. Pada komponen mixture pertama, faktor yang
berpengaruh signifikan terhadap laju kesembuhan pasien DBD yaitu faktor jenis kelamin
dan kadar hematokrit. Sedangkan faktor yang berpengaruh pada komponen mixture
kedua yaitu kadar hematokrit. ========== Dengue Hemorrhagic Fever (DHF) is the most famous tropical disease in
Indonesia, as a country with high rainfall. This disease has been spread by Aedes
Aegypty, a type of mosquito that has been bread in tropical country. This type of
mosquito can be found in many places in Indonesia, included East Java especially
Pamekasan (Madura) which has the high incidence rate of DHF since January 2009. The
number of DHF’s victim rise every year, thousands of people had infected and hundreds
died every year. It is, therefore, necessary to investigate some factors influencing the
patient's healing rate. Factors that influence patient’s DHF recovery include age, gender,
hematocrit’s rate, and platelet ammount. This research employed Bayesian Mixture
Survival Spatial to analyze factors that influence patient’s DHF recovery from
Pamekasan Hospital in Madura, Indonesia. Model includes spatial random effects or
spatial frailty CAR (Conditional Autoregressive) terms to overcome its unexplained
spatial dependencies in the model. In this case, the Queen’s contiguity matrix is used as
weighted matrix. Spatial autocorrelation statistic Moran’s I is used to determine whether
there is any influence of the space/spatial of the sub-districts to the occurrence of the
DHF’s incidence rate in Pamekasan, it show that the statistical Moran’s I equal to
0.1993 and Z-value equal to 2.411 so it can be concluded that there is significant
space/spatial influence in the DHF’s incidence rate in each sub-district in
Pamekasan. 2-parameter weibull distribution is the most appropriate distribution to
modeling DHF patient's healing rate. In this case, there is bi-modal case include the first
mixture component has peak at around t=4 and the second mixture component has peak
at around t=8. Survival function of DHF’s patient was gradually decline along with
patient’s length of hospitalization, but hazard function of DHF’s patient was increasing
gradually. For the first mixture component, factors that could affect the DHF patient's
healing rate, that is patient’s gender and hematocrit’s rate, and factor that influencing
significantly for the second mixture component is hematocrit’s rate.
