Abstract

PT.X adalah perusahaan berkelas dunia yang menghasilkan produk sabun mandi.
Perusahaan mengalami beberapa permasalahan yang berkaitan dengan kualitas produk
sabun. Permasalahan warna sabun diluar spesifikasi merupakan permasalahan yang paling
merugikan perusahaan dan memiliki frekuensi terjadi yang tinggi sehingga menjadi fokus
penelitian. Berdasarkan observasi awal, penyebab utama cacat warna diluar spesifikasi
adalah human error saat proses pencampuran. Langkah awal penelitian adalah dengan
menganalisa kondisi eksisting dan mengidentifikasi akar penyebab masalah menggunakan
root cause analysis(RCA). Human reliability assessment (HRA) dilakukan karena aspek
human adalah penyebab utama permasalahan. Metode HRA yang digunakan untuk adalah
hierarchial task anaysis (HTA), human error assessment and reduction technique
(HEART) dan systematic human error reduction and prediction approach (SHERPA).
Metode tersebut digunakan untuk mengidentifikasi, menganalisis, mengkuantifikasi dan
mereduksi human error yang terjadi. Diperoleh beberapa human error yang memiliki
failure probability besar dan tingkat kekritisan tertinggi. Human error tersebut
diklasifikasikan kedalam kategori kesalahan penuangan dan kesalahan komunikasi. Faktor
yang berkontribusi sebagai penyebab kedua kategori tersebut adalah human dan
management (policy/organization) Rekomendasi perbaikan yang ditawarkan meliputi
training and coaching, perubahan sistem reward and punisment.

PT.X is a world class company that produces soap. Companies are experiencing
some problems related to product quality soap. Color out of specification is a problem that
most harm to the company and have a high frequency occurs so that becme the focus of
research. Based on preliminary observations, the main cause of color out of specification
is human error during the process of mixing. The initial step of this research is to analyze
existing conditions and identify the root cause of the problem using root cause analysis
(RCA). Human reliability assessment (HRA) carried out because the human aspect is the
main cause of the problem. HRA methods used for hierarchial task anaysis (HTA), human
error assessment and reduction technique (HEART) and systematic human error reduction
and prediction approach (SHERPA). The methods used to identify, analyze, quantify and
reduce human error that occurred. There is some human error that has a great probability
of failure and the highest level of criticality. Human error is classified into categories
pouring errors and communication errors. The contributing factors as the cause of both
categories are human, management (policy / organization) and the environment.
Recommended improvements offered include training and coaching, change the reward
system and punisment.
