Abstract

Menurut data WHO, stroke menempati peringkat kedua sebagai penyakit tidak menular penyebab kematian dan peringkat ketiga penyebab kecacatan. Stroke mengakibatkan gangguan neurologis serius, seperti berkurangnya kemampuan motorik anggota tubuh dan otot, kognitif, visual dan koordinasi secara signifikan. Proses monitoring fungsi koordinasi motorik dalam rehabilitasi stroke umumnya dalam bentuk pengamatan kemampuan gerakan. Metode ini memiliki akurasi yang kurang terukur. EEG adalah alat yang mampu memberikan informasi tentang sinyal listrik pada cerebral cortex otak. Fungsi ini bisa dimanfaatkan untuk proses monitoring rehabilitasi stroke terkait fungsi koordinasi motorik. Ditengah pandemic covid19, WSO (World Stroke Organization) melakukan survei didapatkan hasil bahwa terjadi penurunan kunjungan pasien stroke ke rumah sakit. Keterlambatan proses terapi stroke bisa meningkatkan morbiditas dan mortalitas pasien. Terapi stroke home care bisa menjadi salah satu alternative untuk solusinya. Penelitian ini bertujuan untuk mengetahui efektifitas antara terapi stroke home care dibandingkan dengan terapi stroke di Instalasi Rehab Medik Rumah Sakit yang dimonitoring dengan EEG di era new normal. Hipotesis dalam penelitian ini adalah bahwa terapi stroke homecare lebih efektif dibandingkan terapi stroke di rumah sakit di era new normal. Data dalam penelitian ini diperoleh dari hasil pengukuran EEG pada 6 orang pasien stroke selama melakukan tugas gerakan di channel C3 dan C4. Tiga partisipan yang terdiri dari 2 laki-laki dan 1 perempuan melakukan terapi homecare, sedangkan 3 partisipan terdiri dari 2 laki-laki dan 1 perempuan melakukan terapi di rumahsakit. Partisipan melakukan melakukan fleksi-ekstensi bahu. Gerakan dilakukan di sisi tangan yang terefek stroke. Untuk preprocessing sinyal EEG, penelitian ini menerapkan IIR pada tahap filter bandpass. Diikuti oleh algoritma AAR dan ICA untuk menghapus artefak. EEG bersih disegmentasi menjadi 20 ms sebelum menghitung fitur STD . nilai std dianalisis perkembangannya dari terapi pertama sampai terapi ke delapan . Hasil menunjukkan bahwa nilai STD untuk partisipan yang melakukan terapi dirumah lebih banyak yang naik dibandingkan yang melakukan terapi dirumahsakit. Dari penelitian ini, dapat disimpulkan bahwa terapi stroke home care di era new normal lebih efektif daripada terapi stroke di rumahsakit.

According to WHO data, stroke ranks second as a non-communicable disease that causes death, and ranks third as a cause of disability. Stroke causes serious neurological disorders, such as reduced motor skills of limbs and muscles, cognitive, visual and coordination significantly. The process of monitoring motor coordination function in stroke rehabilitation is generally in the form of observing movement abilities. This technique has lower quantifiable accuracy. It is important to employ objective approaches to obtain the appropriate diagnosis and prognosis for effective clinical rehabilitation decisions. Recently EEG has been used as a tool for monitoring stroke rehabilitation especially for motor coordination functions. In the midst of the COVID-19 pandemic, according to the World Stroke Organization survey, it was reported that there was a decrease in visits of stroke patients to hospitals during 2020. Delay in the stroke therapy process could increase patient morbidity and mortality. Home care stroke therapy then becomes an alternative solution. This research is aimed to explore the effectiveness of home care stroke therapy compared to hospital stroke therapy during rehabilitation process using EEG. The EEG from C3 and C4 channels were compared during rehabilitation process of 6 stroke patients when they performed 8 times of rehabilitation program. The motion that was tested is shoulder flexion extension movement on the affected upper limb from each patient. EEG preprocessing was done by applying Infinite Impulse Response for band pass filter and then Automatic Artifact Removal and Independent Component Analysis for artifact removal processing. The cleaned EEG was segmented to 20 ms before calculating the STD feature (Standard deviation). Eight STD values from all rehabilitation program were obtained and analyzed for every patient. The result shows that the STD value of the participants who carried out the therapy at home shows more increase compared to those who did the therapy at the hospital. From this research we can conclude that stroke home care therapy in the new normal era is more effective compared to hospital stroke therapy.
