Abstract
Meskipun protokol terbaru IAEA TRS-398 Rev. 1 sudah terbit, sampai saat ini fasyankes di Indonesia masih tetap menggunakan TRS-398 dan TRS-398 V.12 untuk foton mode with flattening filter (WFF), dan bahkan TRS-483 untuk foton mode flattening filter free (FFF) yang sebenarnya didedikasikan khusus untuk “small static photon fields with energies less than 10 MV”. Garis besar tujuan penelitian ini adalah mengevaluasi perbedaan hasil penerapan keempat protokol tersebut dan memberikan rujukan kepada fasyankes di Indonesia yang belum menggunakan IAEA TRS-398 Rev. 1. Adapun fokus utama penelitian terletak pada perbedaan data dosimetri faktor koreksi kualitas berkas (k_Q ) yang berimplikasi terhadap perbedaan deviasi dosis. Evaluasi dilakukan pada pesawat linac foton sinar-X 6 MV dan 10 MV beserta mode WFF dan FFF dengan menggunakan protokol IAEA TRS-398, TRS-398 V.12, TRS-483 dan TRS-398 Rev. 1. Perhitungan deviasi dosis pada semua variasi pengukuran dan protokol menunjukkan hasil “lolos” (kurang dari 2%) dengan nilai terendah 0.016% dan tertinggi 0.767%, tetapi hasil evaluasi percent diff. k_Q sebesar 0.0803—0.5051% dapat berkontribusi pada percent diff. deviasi dosis sebesar 5.4054—186.2955%. Namun, percent diff. deviasi dosis yang sangat besar (lebih dari 100%) tidak berimplikasi langsung pada dosis serap (D_(w,Q) ). Dengan demikian, penelitian menunjukkan perbedaan nilai k_Q dapat berkontribusi pada hasil deviasi dosis sehingga publikasi ini menyarankan peralihan data dosimetri terbaru sesuai protokol IAEA TRS-398 Rev. 1 bagi fasyankes di Indonesia yang masih menggunakan data dosimetri dari TRS-398, TRS-398 V.12, dan TRS-483 sebagai protokol

Although the updated IAEA TRS-398 Rev. 1 protocol has been published, healthcare facilities in Indonesia still commonly apply the original TRS-398 and its V.12 version for photon beams with flattening filter (WFF), and even TRS-483 for flattening filter free (FFF) photon beams, despite TRS-483 being specifically intended for “small static photon fields with energies less than 10 MV”. This study aims to evaluate the differences in dosimetric results across these four protocols and to provide evidence-based recommendations for Indonesian facilities that have not yet adopted IAEA TRS-398 Rev. 1. The primary focus is the impact of variations in the beam quality correction factor (k_Q ) on dose deviation outcomes. Evaluations were carried out on 6 MV and 10 MV X-ray photon beams in both WFF and FFF modes, using protocols TRS-398, TRS-398 V.12, TRS-483, and TRS-398 Rev. 1. Dose deviation calculations across all measurements and protocol variations showed acceptable results (i.e., deviations below 2%), ranging from 0.016% to 0.767%. However, percent differences in k_Q values ranging from 0.0803% to 0.5051% were found to contribute to dose deviation percent differences as high as 186.2955%. Nevertheless, such large percentage deviations in calculated dose do not directly translate into significant differences in the absorbed dose (D_(w,Q) ). These findings highlight the influence of k_Q on dose calculations and support the recommendation for transitioning to the latest dosimetric data as outlined in IAEA TRS-398 Rev. 1, particularly for institutions still relying on older data from TRS-398, TRS-398 V.12, or TRS-483
