Abstract

Based on the 2021 production plan made by the LPP Department, the ISS Production Department (Injection, Salep and Syrup) PT. Phapros, Tbk will produce products that are dominated by ampoule injection preparations. Currently, in the viewing and labeling process of ampoule injection, waste is found such as defects (broken, cracked, dirty, no batch number, as well as ampoules with damaged batch number), waiting and others. The existence of this waste is quite detrimental to the company in terms of cost and time. Therefore, problem solving and quality control analysis are carried out in an effort to reduce waste in PT Phapros, Tbk using the Lean Six Sigma method which is expected to improve processes, eliminate product or process defects and reduce cycle time and speed up the process. The steps in solving this problem are carried out according to the DMAIC method. The first step to solving the problem is to identify the viewing and labeling process as well as waste using Value Stream Mapping (VSM) and Waste Assessment Model (WAM), DPMO and Sigma Level. Next, an analysis of the causes of the problems was carried out using Cause Analysis 5 Why's and looking for the value of failure risk using Failure Mode and Effect Analysis (FMECA). Finally, an analysis of the design recommendations for improvement will be carried out. From solving this problem, it is expected to reduce waste and cost efficiency in the viewing and labeling process of 2 ml Methyl Prednisolone ampoule injection in the company.
