Abstract
Kegiatan produksi premix pada industri pakan ternak sangat bergantung pada keandalan mesin bagging karena mesin ini berperan sebagai tahap akhir proses pengemasan sebelum produk didistribusikan. Gangguan pada mesin bagging dapat menyebabkan meningkatnya downtime, keterlambatan produksi, serta menurunkan efisiensi operasional perusahaan. Oleh karena itu, diperlukan analisis risiko kegagalan untuk menentukan prioritas perbaikan dan pemeliharaan mesin. Laporan magang ini bertujuan untuk mengidentifikasi mode kegagalan yang terjadi pada mesin bagging, menganalisis penyebab kegagalan, serta menentukan tingkat prioritas risiko menggunakan metode Failure Mode and Effect Analysis (FMEA). Pengumpulan data dilakukan melalui observasi langsung, wawancara dengan operator dan teknisi, serta studi dokumentasi terhadap data kerusakan dan pemeliharaan mesin selama kegiatan magang di PT XYZ melalui PT Bima Sakti Luhur. Selain itu, Weibull Analysis digunakan untuk menganalisis pola kegagalan berdasarkan data waktu kerusakan komponen. Hasil analisis menunjukkan bahwa beberapa komponen mesin bagging memiliki tingkat risiko yang lebih tinggi dibandingkan komponen lainnya sehingga memerlukan tindakan perbaikan dan pemeliharaan yang diprioritaskan berdasarkan nilai Risk Priority Number (RPN). Penerapan metode FMEA yang didukung Weibull Analysis mampu membantu perusahaan dalam mengidentifikasi penyebab utama kegagalan, menentukan prioritas tindakan perbaikan, serta meningkatkan efektivitas program pemeliharaan guna mengurangi risiko kerusakan dan menjaga kelancaran proses produksi.

The premix production process in the animal feed industry highly depends on the reliability of the bagging machine, as it serves as the final packaging stage before products are distributed. Failures in the bagging machine may increase downtime, delay production, and reduce the company's operational efficiency. Therefore, a failure risk analysis is required to determine maintenance and improvement priorities. This industrial internship report aims to identify failure modes in the bagging machine, analyze their root causes, and determine risk priorities using the Failure Mode and Effect Analysis (FMEA) method. Data were collected through direct observation, interviews with operators and maintenance personnel, and documentation studies of machine failure and maintenance records during the internship at PT XYZ under PT Bima Sakti Luhur. In addition, Weibull Analysis was applied to evaluate failure patterns based on component time-to-failure data. The results indicate that several bagging machine components have higher risk levels than others, requiring prioritized corrective actions based on their Risk Priority Number (RPN). The implementation of FMEA supported by Weibull Analysis assists the company in identifying the primary causes of failures, establishing maintenance priorities, and improving the effectiveness of maintenance programs to reduce machine failures and ensure a smooth production process.
