Abstract

To meet the energy demands that Indonesia has, PT. Pertamina Hulu Energi Offshore North West Java (PT. PHE ONWJ) constructs pipelines in an effort to develop and connect upstream and downstream oil production efforts. One of the operations that is part of the subsea pipeline construction process is the offshore pipelaying procedure which employs a range of equipment as part of the operational procedure. These equipments used in the operation have their own risks and failure modes that could happen as the operation is underway, which is why this research is conducted to find out which equipment used in the offshore pipelaying procedure needs to be prioritized based on the traditional failure mode and effects analysis (FMEA) which is qualitative in nature, and the probabilistic FMEA which is quantitative in nature. Based on the research that has been done, the equipment that needs to be prioritized based on the qualitative traditional FMEA method is the anchor handling tug (AHT) with its engine breakdown failure mode that has a risk priority number (RPN) score of 27.29. While based on the quantitative probabilistic FMEA method, the pipelaying barge mooring system is the equipment that needs to be prioritized during the offshore pipelaying operations for its undetected failure mode (P(uf)), which is a mooring failure, has a 0.0001% chance of happening.
