Abstract
Terminal Bahan Bakar Minyak (TBBM) Pertamina Tuban merupakan salah satu dari sekian banyak fasilitas milik PT. Pertamina yang digunakan untuk pendistribusian bahan bakar minyak. Dalam pengoperasiannya, distribusi minyak dilakukan menggunakan tanker. Terdapat dua SPM jenis CALM (Catenary Anchor Leg Mooring) milik TBBM Pertamina Tuban. Salah satunya SPM 035 Pertamina memiliki kapasitas tambat maksimal sebesar 35.000 DWT. Mulai Desember 2011, SPM 035 Pertamina mulai digunakan juga oleh tanker dengan kapasitas 50.000 DWT. Hal ini membuat SPM 035 Pertamina memiliki banyak risiko, khususnya mooring system. Untuk melakukan pengelolaan terhadap risiko yang terjadi maka penelitian ini menggunakan metode Integrated FTA-FMEA. Dari hasil analisis risiko yang dilakukan, didapatkan risiko pada sembilan failure mode yaitu, mooring lugs patah sebesar Rp. 82.884.824,00, shackles terlepas sebesar Rp. 54.319.997,00, hardwood battens pecah sebesar Rp. 23.107.188,00, mooring briddle tidak berfungsi sebesar Rp. 101.065.104,00, mooring hawser shackles rusak sebesar Rp. 31.007.052,00, triangular plate pecah sebesar Rp. 103.186.521,00, mooring hawser terputus sebesar Rp. 406.520.199,00, chain support buoy sebesar Rp. 54.840.078,00, dan samson buoy rusak sebesar Rp. 60.997.158,00. Adapun program perbaikan pada risiko kritis dengan menambahkan dua buah mooring hawser atau dengan mengganti mooring hawser yang lebih besar

Fuel Terminal (TBBM) Tuban Pertamina is one of the many facilities owned by PT. Pertamina used for distributing fuel oil. In operation, the distribution is done using oil tankers. There are two types of SPM CALM (Catenary Anchor Leg Mooring) owned by Pertamina TBBM Tuban, respectively - each have different capacities. One of them SPM 035 mooring Pertamina has a maximum capacity of 35,000 DWT. Starting in December 2011, SPM 035 Pertamina began to be used also by tankers with a capacity of 50,000 DWT. This makes SPM 035 Pertamina has many risks, especially the mooring system. To take over management of the risk occurring, this study used methods Integrated FTA-FMEA.
