Abstract
Hearing screening dapat dilakukan dengan menggunakan audiometer portabel 3 AFC yang memungkinkan pemeriksaan jarak jauh secara mandiri di ruang terbuka, salah satunya bagi para penderita gangguan pendengaran yang berkaitan dengan usia (age-related hearing loss) yang umum dialami pada orang lanjut usia. Tujuan pada penelitian ini adalah menganalisis perbandingan kinerja audiometri portabel 3 AFC dan audiometri gold standard melalui uji statistik dan diagnostik. Penelitian ini mengikutsertakan 39 subjek lansia, di mana setiap subjeknya telah diuji dengan audiometer gold standard di Poli Audiologi RSUD Dr. Soetomo Surabaya dan audiometer portabel 3 AFC di rumah masing-masing subjek dengan perulangan sebanyak lima kali. Uji audiometri gold standard menghasilkan 58% dari total subjek memiliki kondisi pendengaran normal (≤25 dB HL) dan 42% lainnya memiliki kondisi gangguan pendengaran (>25 dB HL), sedangkan uji audiometri portabel 3 AFC mendeteksi 64% dari total subjek memiliki kondisi pendengaran normal (≤25 dB HL) dan 36% lainnya memiliki kondisi gangguan pendengaran (>25 dB HL). Berdasarkan aturan Occupational Safety and Health Administration (OSHA) mengenai batas Significance Threshold Shift (STS), pada frekuensi 250, 500, 1000, dan 2000 Hz jumlah pergeseran nilai ambang dengar antara audiometer gold standard dan audiometer portabel 3 AFC yang lebih dari 10 dB berjumlah kurang dari 50%. Sedangkan dari uji diagnostik kemampuan audiometer portabel 3 AFC untuk mengklasifikasikan seseorang yang sakit (sensitivity) adalah 82% dan untuk kemampuan audiometer portabel 3 AFC untuk mengklasifikasikan seseorang yang tidak sakit (specificity) adalah 87%.

Hearing screening can be performed using a portable 3 AFC audiometer that allows for remote testing in open spaces, especially for those who suffer from age-related hearing loss that is commonly experienced by the elderly people. The aim of this study is to analyze the performance comparison between the portable 3 AFC audiometry and the gold standard audiometry through statistical and diagnostic tests. This study included 39 elderly subjects, each of whom was tested with a gold standard audiometer at the Audiology Clinic of Dr. Soetomo Surabaya Hospital and a portable 3 AFC audiometer at their respective homes with five repetitions. The gold standard audiometry test resulted in 58% of the total subjects having normal hearing conditions (≤25 dB HL) and 42% having hearing impairment conditions (>25 dB HL), while the portable 3 AFC audiometry test detected 64% of the total subjects having normal hearing conditions (≤25 dB HL) and 36% having hearing impairment conditions (>25 dB HL). Based on the Occupational Safety and Health Administration (OSHA) rule regarding the Significance Threshold Shift (STS) limit, at frequencies of 250, 500, 1000, and 2000 Hz, the number of shifts in hearing threshold values between the gold standard audiometer and the portable 3 AFC audiometer that were more than 10 dB was less than 50%. Meanwhile, from the diagnostic test, the ability of the portable 3 AFC audiometer to classify someone who is sick (sensitivity) is 82% and the ability of the portable 3 AFC audiometer to classify someone who is not sick (specificity) is 87%.
