Abstract

Gagal ginjal kronis (GGK) merupakan penurunan fungsi ginjal yang berlangsung lebih dari tiga bulan. Penyakit ini telah menjadi masalah kesehatan tidak hanya di Indonesia, tetapi juga di seluruh dunia. Banyak upaya pencegahan GGK yang telah dilakukan, mulai dari pendekatan medis hingga spiritual. Dengan adanya metode komplementer, termasuk intervensi spiritual, penderita GGK dapat lebih bersemangat dan menerima penyakitnya sehingga membantu proses perbaikan klinis mereka. Rumah Sakit Nur Hidayah Bantul telah menerapkan Hu-Care dalam pendekatan spiritualnya. Hu-Care merupakan metode yang menggabungkan konsep husnul khatimah dan perawatan paliatif. Penelitian ini memodelkan pengaruh intervensi spiritual Hu-Care beserta faktor pendukung terhadap laju kekambuhan penyakit pada pasien GGK yang dirawat inap di rumah sakit tersebut. Penelitian ini menggunakan metode Extended Cox Regression karena variabel riwayat penyakit penyerta hipertensi dan anemia tidak memenuhi asumsi proportional hazard. Model terpilih adalah stratified Cox dengan dua variabel stratifikasi dengan nilai AIC sebesar 120,6423. Model terbaik didapatkan dengan variabel jenis kelamin sebagai variabel prediktor dengan hazard ratio sebesar 0,431 dengan interval 0,2902 - 0,8890 pada selang kepercayaan 90%. Hasil tersebut menunjukkan bahwa kondisi psiko-spiritual pasien yang menjadi topik bahasan penelitian ini tidak berpengaruh signifikan dalam model yang didasari pada kurangnya inklusivitas dan panduan yang jelas dalam proses asesmen Hu-Care.

Chronic kidney failure (CKD) is a long-lasting decline in kidney function resulting from kidney damage that persists for at least three months. It is a prevalent global health concern, including in Indonesia, and efforts to prevent and reduce CKD cases encompass medical and spiritual approaches. While medical treatments can manage the severity of chronic diseases like CKD, they do not guarantee complete recovery. Complementary methods, including spiritual interventions, have emerged to address the physical and medical aspects of CKD. Nur Hidayah Bantul Hospital in Indonesia has implemented Hu-Care, a combination of husnul khatimah and palliative care concepts. This study employs Cox regression modeling to examine how Hu�Care's spiritual intervention affects disease recurrence in hospitalized CKD patients. This study use the Extended Cox Regression method because the variable historical of hypertension and anemia violate the proportional hazard assumption. The selected model is stratified Cox with two stratified variables with an AIC value of 120.6423. Best model were obtained with gender as a predictor variable with a hazard ratio of 0.431 with 0.2902 - 0.8890 interval in confidence interval 90%. These results indicate that the patient's psycho-spiritual condition which is the topic of this study has no significant effect on the model based on the lack of inclusiveness and clear guidelines in the Hu-Care assessment process. Therefore, it is necessary to re-evaluate the assessment process related to the question instrument and the data collection process.
